POSITION SUMMARY:

Under the Supervision of the Director of Integrated Health, theFamily and Community Engagement Coordinatorwill be responsible for the planning, implementation and oversight of family support services, community partnerships, and all elements of the enrollment and attendance process. The Family and Community Engagement Coordinator for the Parents as Teachers (PAT) program is responsible for providing in-home support, education, and resources to families with young children. This role focuses on enhancing child development and family well-being through regular home visits and community engagement.

ESSENTIAL DUTIES AND RESPONSIBILITES:

T he duties below are intended to provide an overview of the duties required of the Family and Community Engagement Coordinator.

  • Home Visits:

  • Conduct regular home visits to provide parents with child development information, parenting support, and resources.

  • Develop and implement individualized family service plans based on assessments and family goals.

  • Parent Education:

  • Facilitate parent education sessions using the Parents as Teachers curriculum.

  • Provide guidance on child development, health, and safety practices.

  • Family Support:

  • Offer emotional support and practical assistance to families.

  • Connect families with community resources and services as needed.

  • Assessment and Planning:

  • Conduct developmental screenings and assessments for children.

  • Collaborate with families to set and achieve goals for their children’s development and family well-being.

  • Conduct developmental screenings and assessments to monitor child development and identify any concerns.

  • Program Coordination:

  • Schedule and coordinate home visits and group meetings.

  • Maintain accurate records of family interactions, assessments, and services provided.

  • Community Liaison:

  • Build and maintain relationships with community agencies and service providers.

  • Act as a liaison between families and community resources.

  • Training and Development:

  • Participate in ongoing training and professional development related to the Parents as Teachers program.

  • Stay updated on best practices in early childhood education and family support.

  • Documentation and Reporting:

  • Maintain organized records of family interactions and services provided.

  • Prepare and submit reports as required by program management.

  • Advocacy:

  • Advocate families within the community and with service providers to ensure they receive necessary support.

  • Crisis Intervention:

  • Provide immediate support and intervention in crisis situations, helping families navigate challenges and access emergency resources.

  • Professional Development:

  • Participate in ongoing training and professional development to stay current with best practices in family support and early childhood education.

  • Collaboration:

  • Work collaboratively with other professionals and service providers to coordinate care and support for families.

Qualifications

CREDENTIALS, ABILITIES AND EXPERIENCE:

  • Education: Bachelor's degree in social work, human services, early childhood education, or a related field.

  • Experience: Experience in social work, family support, or a related field, preferably with children and families.

  • Knowledge: Strong understanding of child development, family dynamics, and community resources. Familiarity with the PAT model is a plus.

  • Skills: Excellent communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Proficiency in using virtual platforms and basic computer applications.

  • Language: Bilingual skills (e.g., Spanish) are highly valued.

  • Other Requirements:

  • Valid driver's license and reliable transportation.

  • Successful completion of PAT Foundational and Model Implementation Training.

  • Ability to effectively manage time and be flexible to the demands of the position.

  • Ability to demonstrate initiative and resourcefulness, and to safely and competently respond to urgent and emergent situations (within the scope of position).

  • Ability to maintain professional boundaries with the other employees and with children and families.

  • Ability to maintain confidentiality of all records and information that pertain to children, families, employees, agency, and its related interests.
