The centers provide campus-based direct services and supports at 13 centers located throughout the state. Work involves performing routine consultative services and providing technical assistance during monitoring of individuals following their move from a State Supported Living Center (SSLC) to a community setting. The PMM assists in the development of supports and services as part of the transitioning planning process. Observes and interacts with individuals and community provider staff to determine if the supports and services identified in the transition plan are in place and effective for the individual. The PMM completes a written report and notifies SSLC staff of any identified issues. Mediates disputes/dissatisfaction with community supports and services following transition from the facility and works to develop solutions for identified issues. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment. This position is required to travel up to 50% of the time.Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.Provides support and collaboration in the planning and development of community supports as part of the transition planning process for persons moving from the SSLC to a community setting. Schedules, coordinates, and conducts on-site monitoring of individuals following their move from the SSLC to a community setting.
