Responsibilities

JOB SUMMARY: The transfer nurse assists the Shift Triage Coordinator in completing transfer requests from referral sources in accordance to EMTALA and facility guidelines.

Key Responsibilities :

  • Receives all inquiry calls and responds to requests for information and assessments rapidly and ensures relevant report is obtained and passed on to receiving unit/physician.

  • Expert in documentation, communication, teamwork and customer service. Advocates for internal and external customers including the client, family, physician, medical facility, behavioral healthcare facility and managed care organizations.

  • Liaisons with physicians, psychiatrist, other facilities and managed care organizations to promote optimal use of the continuum of care for each individual transferred.

  • Enters relevant information into EMR for patients arriving on shift requiring admission. This includes, but is not limited to order entry for Telephone orders given by admitting physician and medication entry.

    Qualifications

    QUALIFICATIONS :

    SKILLS, KNOWLEDGE AND ABILITIES: Knowledge of psychiatric behavioral health diagnostic criteria (DSM IV/V), quality assessment skills, including listening and writing legibly, ability to provide customer service in a caring way, even in a high stress environment. Excellent verbal and written communication skills. Knowledge of COBRA/EMTALA laws.

    EDUCATION: Graduate of an accredited nursing school. LICENSURE/CERTIFICATION: Texas license as an RN is required. EXPERIENCE: 1 year mental health experience. Preferred: Prior experience as a supervisor in transfer coordinator.

    PHYSICAL REQUIREMENTS:

  • Must be able to lift and carry up to 20 lbs

  • Occasional bending, stooping and kneeling

  • Must be able to sit for extended periods of time at computer terminal

  • Must have hearing and vision sufficient to perform essential activities

  • Must have ability to distinguish letters and symbols for reviewing clinical documentation

  • Must be able to work under stressful conditions

  • Must have excellent verbal and written skills

  • Must show emotional stability necessary to perform the essential functions of the job
