Within the framework of recovery and resiliency provides client-centered therapy to individuals in the Youth Adult Transitional program.
ESSENTIAL FUNCTIONS:
Completes timely biopsychosocial assessments of individuals admitted to the YAT.
Collaboratively develops treatment plans and monitors progress of goals.
Responsible for coordination of care with internal/external providers and naturals supports.
Creates and implements comprehensive discharge/aftercare plan for each individual.
Ensures proper releases and service agreements are acquired.
Ensure documentation is timely and reflects medical necessity.
Attends treatment team and service planning meetings when requested.
Performs all other duties as assigned.

Demonstrates at all times the commitment to providing the least intrusive service intervention that respects the clients’ needs/preferences in the least restrictive setting that supports client and staff safety.
Communicates with all colleagues in a respectful, professional, responsive manner that promotes teamwork and supports positive outcomes for clients.
Uses a consistent problem-solving focus in working to resolve differences with colleagues and clients.
Ascertains that relevant information is reported to program manager and/or the team in a timely manner.
Maintains current knowledge in present practice area, i.e., attendance at required hours of organization educational programs and utilizes clinical supervision as a learning experience.

Reports accidents and injuries in a timely fashion, demonstrates safe housekeeping practices, notifies supervisor of potential health and safety concerns immediately, participates in health and safety training as required, understands and follows all Sweetser health and safety policies.
Ability to support and recognize the important role volunteers play. Sweetser employees work with, support, collaborate with volunteers, and promote opportunities for volunteer engagement.

EDUCATION:
- Master’s degree in either social work, psychology, clinical counseling or other comparable mental health program.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
LCSW, LCPC or LMFT, Conditional licensure.
Maintain a valid state driver’s license.

Recognizes signs of increasing clinical or situational risk and of escalating behavior and utilizes de-escalation and safety implementation techniques effectively.
Recognizes variables underlying stressful situations, discerns methods to diffuse or contain them and quickly implements the most effective intervention.
Provides crisis/risk assessments and develops/implements stabilization plans that are creative and specific to the persons served and their needs, and promotes health and growth with the goal of reintegration into the community
