Performs advocacy, education, coordination of care, and counseling services for oncology patients. Supports team, system and organizational goals. Uses critical thinking skills to obtain best possible outcomes. Promotes lifelong learning and evidence-based practice to improve the care of patients with a past current or potential diagnosis of cancer. Leads Commission on Cancer Accreditation efforts. Participates in screening events in facility. Collaborate with community partners. Navigate patients through the continuum of care.

Essential Functions:

  • Participates in assessing, planning, implementing, and evaluating oncology health services.
  • Responsible for developing and overseeing the care coordination provided to the client and is liaison between the MD, professional health care staff and others involved parties.
  • Coordinates care for patients from a variety of ethnic, cultural and socioeconomic backgrounds and with varied medical and developmental needs.
  • Communicates and collaborates with members of the interdisciplinary healthcare team.
  • Follows up on team recommendations and assists in care coordination to promote timely follow up and treatment.
  • Assesses and identifies high risk patients (physical, psychological and social).
  • Assess educational needs of patient, family and caregivers taking into consideration barriers to care.
  • Facilitates individualized care based on assessment, patient needs and preferences.
  • Orients and educates patients, families and caregivers to the healthcare system, and team member roles.
  • Provides psychosocial support and facilitates appropriate referrals especially during periods of high emotional stress and anxiety.
  • Identifies/implements and evaluates the education plan based on patient’s assessed need.
  • Empowers patients and families through education and encouragement to self-advocate and communicate their needs.
  • Demonstrates appropriate and assertive methods of communication.
  • Promotes an interactive relationship that helps communication among patient, family and nurse.
  • Ensures’ patient’s right to choice.
  • Assists with transition from active treatment to survivorship or end-of-life care.
  • Builds an awareness of the Oncology Nurse Navigator role through relationships with team members, community and patients.
  • Identifies medical staff and community needs for education and services.
  • Collaborates with other educators to provide educational offerings.
  • Reports outcomes and participates in quality improvement initiatives.
  • Ensures documentation of patient encounters and provided services.

Minimum Education

  • Associates Degree
  • Bachelor's Degree in relevant clinical service preferred.

    Required Skills

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

  • Must be able to work in a stressful environment and take appropriate action.

  • Current license to practice in NC for relevant field.
