University of Rochester Thoracic and Foregut Surgery RN Coordinator - RN, Nurse Navigator in Rochester, New York
Responsibilities
A new and unique position for a motivated and self-directed RN that is passionate in complex care coordination, educating and advocating for patients. The RN Coordinator role includes the development and collaboration with internal and external relationships required for complex care coordination, examples include but are not limited to early home care identification, vendor support, and nutritional monitoring support. The RN coordinator will work closely with the Thoracic Nurse Navigator to work towards early identification of high risk needs patients. Additional responsibilities include call triage, disability paperwork, and prior authorization documentation. Time management and good organization skills necessary to ensure coordination of resources. Strong communication skills essential for successful role as advisor and liaison between patients and physicians. A minimum of 3 years required. Experience in Complex Care Coordination, Thoracic and Foregut Surgery and oncological care preferred but not required. Reports directly to clinic supervisor. Works under the administrative direction of the Lead APP and the clinical direction of the Thoracic and Foregut physician directors.
Qualifications
Bachelor in Nursing required; if currently an Associate's Degree RN, nurse has 5 years to complete the BSN. Three to Five years of Nursing experience required or equivalent combination of education and experience. Required competence in critical thinking, interpersonal relationships and technical skills. Licensure as a Registered Nurse in NYS; certification in a Nursing specialty area preferred.

Pay Range
Pay Range: $36.73 -$47.75
The referenced pay range represents the minimum and maximum annual salary for this faculty position. Individual salaries will be determined within the job’s salary range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Incentive-based compensation may also be applicable and will be discussed during the hiring process.
