Salary Range: $75,000 To $85,000 Annually

RPH is seeking an RN to take on a leadership role as the RN Memory Care Case Manager at our Creekstone Memory Care Residence located at 20 Crossing Creek Drive in Fairport. Job Description
The RN Memory Care Case Manager is responsible for case management services to include initial and periodic evaluation of the needs of each resident and developing individualized wellness plans to meet those needs. The RN is empowered to act on behalf of the Rochester Presbyterian Home (RPH) during the absence of the Administrator concerning the overall health and safety, comfort, and well-being of residents. RPH is licensed and certified by the NYS DOH to provide ALR, SNALR, EALR levels of care. In partnership with the Administrator of Record, this position will oversee care staff and ensure that the physical, health, emotional, spiritual, and social needs of the residents are maintained on an individual basis.
Qualifications
Current NY State licensure as a Registered Nurse in good standing required. Bachelor’s degree from an accredited college or university preferred with major work in the areas of nursing, human services, or service delivery. 3 - 5 years’ full-time management/leadership experience in the provision of services to a dependent adult population required.
