University of Rochester Primary Care Network (Hornell/Bath NY) - Nurse Practitioner/Physician Assistant in Rochester, New York
Responsibilities
Exciting opportunity to join a busy Family Medicine medical practice in the Hornell & Bath, NY area and become part of an interdisciplinary team that provides patient care utilizing the Patient Centered Medical Home model.
Responsibilities include: evaluation and management of acute/chronic illnesses, ordering/interpreting/communicating diagnostic tests and results, complete physical exams, Wellness visits, hospital/ED follow-ups and Transitional Care Management, collaboration with network based physicians and medical team regarding patient care, facilitating appropriate referrals to specialists or community-based agencies, opportunities for office based procedures, patient education/counseling intended to maintain or restore physical/emotional health and function, and educational support for various learners consistent with the goals of the UR teaching institution. Ultimately, the APP will have the opportunity to establish a team supported patient panel.
Excellent communication and collaboration skills are essential with the ability to exercise independent judgement and initiative. The candidate must have completed NYS NP/PA certification along with licensure and have applied for DEA registration prior to hire/start date.
Qualifications
Nurse Practitioner
Requires a Master's Degree in Nursing or related field and an active New York State license in good standing. Applicants will have 5 years experience as an RN with one to three years of leadership experience; or equivalent combination of education and experience. The Nurse Practitioner influences the quality and delivery of care in various roles including consultant, researcher, mentor, and practitioner. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
Physician Assistant
Completion of an AMA approved Physician Assistant program, licensure in good standing, and certified as a Registered Physician Assistant. Demonstrated clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.

Pay Range
Pay Range: $44.71 - $67.07
The referenced pay range represents the minimum and maximum annual salary for this faculty position. Individual salaries will be determined within the job’s salary range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Incentive-based compensation may also be applicable and will be discussed during the hiring process.
