Responsibilities

Clinical Responsibilities:

  • Collaborate with interdisciplinary healthcare teams to guide appropriate medication use, working closely with physicians, nurses, and other professionals to ensure optimal patient care.

  • Drive improved patient outcomes by participating in focused clinical initiatives such as pharmacokinetic consultation and antimicrobial stewardship, while actively contributing to Medication Use Evaluations to optimize therapy.

  • Document clinical activities, interventions, and regularly share your expertise through education, empowering our pharmacy staff and healthcare professionals. Identify and seize opportunities for patient teaching to provide necessary guidance for better patient understanding and outcomes.

  • Utilize your clinical judgment and available resources to approve medication orders, actively contribute to the preparation and dispensing of medications, and supervise our pharmacy technicians for a seamless workflow and accurate medication distribution while ensuring compliance with state and federal laws and regulations.

Problem Solving:

  • Demonstrate proactive problem-solving skills, ensuring thorough resolution of patient care issues through effective communication with relevant staff.

  • Take a leadership role in identifying and addressing patient care problems, avoiding the transfer of issues to the next shift without making reasonable resolution attempts and communicating progress effectively.

  • Engage in problem-solving beyond individual patient cases, actively participating in interdisciplinary teams and contributing to the development or revision of hospital and departmental policies, procedures, and treatment guidelines. Also assume charge pharmacist responsibilities, including coordinating shift resources, resolving staff conflicts, and facilitating communication between shifts. Seek appropriate assistance from supervisors when faced with solutions beyond your expertise or authority.

Qualifications:

  • Bachelor of Science degree in Pharmacy or Pharm D preferred.

  • Possession of a New York State Pharmacy License.

  • Candidates should have either more than one year of relevant acute care pharmacy experience or have successfully completed an accredited pharmacy practice residency.

Pay Range

Pay Range: $107,910 - $161,907 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
