HCR Home Care LPN - In Office- Face to Face Coordinator - Fulltime in Rochester, New York

Role and Responsibilities

This position coordinates and ensures compliance to the Face-to-Face client visit with their primary care physician requirement. This role involves coordination and follow-up with clinical staff and physicians.

Essential Functions

  • Process Workflow in HCHB to produce Face-to-Face documents for physician signature, in accordance with the regulations.

  • Run reports and monitor compliance of returned forms and review for adequacy of documentation to meet the intent of federal regulations.

  • Coordinate with physicians/physicians office staff and HCR clinical staff, as needed, to procure forms not completed.

  • Review returned documentation to ensure clinical components of the requirements are met.

  • Process documents through HCHB to allow billing to occur.

  • Other duties as assigned.
