Starting Pay Rate - $23.50 hourly

The Intake Coordinator will be responsible for the intake process for the Waiver Service Coordination for both Nursing Home Transition and Diversion and Traumatic Brain Injury Waivers. This position will be responsible to respond to all referral emails from the waiver offices, field calls from people interested in services with ESLC, enroll applicants to both waivers in a timely manner, and act as a service coordinator for newly enrolled participants until they can be transitioned to a service coordinator.
ESSENTIAL JOB FUNCTIONS
Enroll approved participants for waiver programs
Complete all required documents for the initial application packet, including the service plan, plan for protective oversight, waiver rights, provider selections, and additional service-specific assessments.
Ensure new applicants meet program level of care through the accepted assessment process (Uniform Assessment System completed by a qualified RN).
Complete any corrections needed for application packets in a timely manner.
Initiate services as approved in the initial application packet.
Maintain referrals and waitlist for eligibility assessment into the programs
Assist with enrollment, coordination, and scheduling of requested and approved waiver services
Partner with Director of Service Coordination to attend functions for referral sources and/or program information, i.e., GRAPE, health fairs, Nursing Homes, Hospitals, Senior Living Facilities, Local DHS agencies, and other possible referring agency
Transition cases for continuation of services by introducing the participant to an ongoing service coordinator and passing on all relevant information to ensure continuity of care.
Schedule required and requested meetings between participant, natural supports, service providers, and any other designated individual as needed.
Document in-person and telephone encounters in the case note and complete all follow up activities including encounters with the participants, or other supports.
Assist with referrals for indicated services (e.g., social services such as housing assistance, vocational rehabilitation, substance abuse treatment, day program, medical doctors).
Requirements
QUALIFICATIONS:
Must have a Valid New York State driver’s license with a clean driving record.
Strong internal and external communication skills
Good Organizational Skills
Flexible, yet organized, with the ability to exercise independent, sound judgment.
Knowledgeable about all community resources, Medicaid Services, and available non-Medicaid services.
Demonstrated ability to collaborate effectively in a team setting.
Ability to maintain effective and professional relationships with patient and other members of the care team.
Strong communication skills.
Ability to effectively engage patients in a therapeutic relationship, when appropriate.
Ability to work with patients by telephone or in person.
Working knowledge of differential diagnosis of common mental health and/or substance use disorders, when appropriate.
Be able to assess patient’s ability to remain in home with or without assistance and determine what level of assistance is needed to remain living in the community
Experience with Microsoft Suite of Products and Email
EDUCATION and EXPERIENCE:
Associate degree in a Health and Human Service field required and a minimum of 2 years of experience in case management employment.
Bachelor’s degree in health and human service field preferred and a minimum of 1 year of experience in case management employment.

Performing duties of this job requires occasional walking and standing in and around the reception area, and lobby. Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required.
