Job Summary and Qualifications
Functions within the nursing care guidelines established by the Director and is accountable to her/him for the actions and results achieved. In collaboration with the Director, assists in the management of daily operations of the department. Develops, plans, and implements quality assurance and educational programs to address identified needs. Performs within the scope of RN practice as outlined in the standards manual and by applicable regulatory and professional ethics/guidelines, standards, protocols and procedures. Collaborates with leadership teams to deliver quality care and excellent customer service for the service line. Collaborates with the Director to address the special needs of the geropsychiatric nursing staff and problems/incidents of a management or administrative nature.. Essential Position Functions:
Clinical Nurse Leader:
Demonstrates a positive attitude that promotes a collaborative atmosphere and a patient focused environment.
Assists in the investigation of clinical and personnel issues as director by the Director of the given unit/department.
Performs all duties in a professional manner, maintaining patient dignity, confidentiality, and safety.
Complies with hospital policies and participates in the development, implementation, and revision of written standards, policies and procedures for Nursing.
Develops and oversees a daily staffing plan that adheres to the staffing guidelines for the unit and supports patient care and patient/staff safety .
Reviews and approves PTO requests and enters payroll.
Participates in interdisciplinary treatment rounds.
Communicates with other departments, physicians, and patient clientele to encourage and foster participation and cooperation to meet patient needs.
Delegates duties to professional and non-professional nursing personnel based upon patient needs, available staff, and total unit needs.
In collaboration with the Program Director, participates in the hiring of staff, completes performance evaluations and conducts individual review sessions annually, coaches staff to improve work performance and to provide high quality service.
Coordinates and evaluates nursing activities directly and indirectly related to patient care.
Reviews nursing documentation for accuracy and adequate recording of patient's condition, plan of care, nursing interventions and discharge needs as appropriate.
Collaborates with Director in performance evaluation of staff.
Develops and maintains appropriate records and reports which reflect the activities of the unit.
Coordinates educational activities to address the education and training needs of the staff.
Supports and assists the staff in their participation in departmental quality improvement activities.
In collaboration with Program Director, develops and implements GPU orientation program and oversees the delivery of education to GPU staff.
Participates in the development and implementation of the departmental operating plan, achieving quality and patient satisfaction outcome goals, and financial stability.
Pursues activities which will promote professional growth and development of self and contribute to the objectives of the department and division.
In collaboration with Program Director, establishes, monitors and facilitates unit based QA & I Program.
Evaluates, investigates and initiates a plan of action related to patient complaints and incident reports.
Intake:
Oversees intake/referral process for inpatient geropsychiatry unit.
Responds to requests quickly, requesting any needed supporting documentation in a timely manner.
Demonstrates diligent, focused, daily effort to maintain maximum census, limit open beds, and to reduce wait times for admission to the inpatient geropsychiatry unit.
Obtains all required clinical/legal referral documentation from referral source and makes clinical determination in conjunction with Program Director, as needed.
Coordinates intake date/time/receiving unit with referral source, GPU staff and FMH staff.
Care Management/Utilization Review
For patients with reviews patient medical records for appropriateness of admission and/or continued stay using established criteria.
Provides requested information regarding patient care and services to external review and regulatory agencies.
Consults with GPU medical providers, Program Director and social workers to assess psychosocial needs of patients and families and to evaluate available resources to help drive interventions aimed at meeting the individualized plan of care and to overcome barriers to timely discharge
Explores and implements strategies to reduce length of stay when appropriate and to optimize resource consumption.
Collaborates with Program Director, medical providers, patient accounts, and other related parties on issues of continued stay, denials and related activities.
Communicates to patients, GPU medical providers ,Program Director, and nurses any changes in review findings resulting in level of care changes and findings consistent with termination/denial of benefits.
Coordinates and performs all follow up activities related to Medicare other third party payor, and hospital negotiated contracts.
Assists medical staff in responding to appeals.
Provides instruction/education to patients/families regarding appeals process upon receipt of Medicare termination of benefits letter.
Maintains close communication with the Program Director regarding utilization, quality of care, risk, and infection control issues.
Informs Program Director and CCO/VP of Patient Care Services about matters that require notification or administrative action.
Ensures the security/confidentiality of patient information.
Documents communication with payor sources and communicates authorization status as appropriate.
Works with members of the interdisciplinary team and community resources to improve responsiveness, capabilities, efficiency and efficacy of admission and discharge process.
Investigates patient rights/issues/referrals. Completes investigation and documentation upon grievances
Education & Experience:
Registered Nurse from an accredited school of nursing with a current, valid license in the State of New Hampshire or Compact State
Bachelor's Degree in Nursing preferred.
Active BLS
Active Nonviolent Crisis Intervention within 90 days
5 or more years' clinical experience.
Prior supervisory or leadership experience strongly preferred.
Advanced preparation in clinical specialty or management is desirable.
Must possess a broad understanding of both theory and clinical practice.
Ability to prioritize work and work independently with minimal supervision.
Ability to set priorities, exercise initiative and judgement and make decisions within scope of assigned authority.
Ability to communicate effectively both verbally and in writing to a diverse audience.
Demonstrated teaching ability required
Ability to handle stressful situations and to manage multiple priorities.
What qualifications you will need:
Basic Cardiac Life Support must be obtained within 30 days of employment start date
Nonviolent Crisis Intervention must be obtained within 90 days of employment start date
