CAROLINE CENTER INC Day Services Coordinator in RIDGELY, Maryland
The Day Services Coordinator (DSC) is a mid-management position responsible for the coordination and oversight of the following DDA-authorized services and supports: Day Habilitation, Community Development Services and Career Exploration. The DSC provides direct supervision to assigned Managers, Leaders and Direct Service Providers (DSPs). The DSC is directly supervised by the Director of Meaningful Day Services.
Status:Exempt The primary hours for this position are 7:30a-3:30p, Monday - Friday. However, this is a salaried mid-management position and hours will need to be flexible based on the demands of the job,
Requirements: Bachelors degree in human services field preferred; AA degree or three (3) years of relevant experience. Preference may be given to applicants with supervisory experience or similar DDA-accepted Employment Specialist credentials.
Must successfully complete certification as an Employment Specialist, via a DDA-approved agent within 90 days of appointment to the position and satisfy all continuing learning requirements to maintain certification; obtain Certification as a Medication Technician (CMT) through the Maryland Board of Nursing,
Possess a valid drivers license and driving record acceptable to the agencys insurance carrier, compassion and respect for people with developmental disabilities, able to: to sit and stand for long periods of time, lift a minimum of 50 lbs. independently.
Communicate articulately using written and verbal skills in the English language and follow instructions with minimal direct supervision.
In this mid-management position general duties are as follows:

- Facilitates and coordinates the development of Program Participants Person-Centered Plans (PCPs),
- Provides direct supervision to assigned DSPs, Leaders
- Participates in and/or facilitates meetings as assigned
- Role models professional supervisory skills
- Demonstrates knowledge of the strengths, needs, preferences and desires of program participants
- Successfully completes all Developmental Disabilities Administration (DDA) and agency required trainings
- Participates in continued learning opportunities to stay abreast of Best Practices in
- Meaningful Day Services for adults with I/DD
- Accurately conveys program needs to the Sr. DOS-East
Specific Duties:
Manages routine functions of assigned Meaningful Day Supports and Services
Conducts assessments of persons referred for services to evaluate ability to safely meet needs in conjunction with other agency leadership, as indicated, and as directed by the DIrecot of Meaningful Day services.
Ensures that activity plans and schedules include opportunities for inclusivity of people without disabilities in non-institutional-like settings
Ensures the development of individualized activity plans and schedules for each Participant that includes estimates of all activities, length of time and place
Develops and/or ensures the development of, program-specific goals and implementation plans for inclusion in the PCP
Participates in PCP Meetings as scheduled, or ensures representation from Day Services
Monitors progress related to program participants goals and implementation plans as outlined in the PCP packet
Monitors program participants attendance forms for accuracy and enters into PCIS in accordance with agency policies and procedures, and copies to CFO by 5th of each month
Promotes effective and efficient communication between departmental staff and other agency departments
Ensures adequate program staffing to meet participants needs
Participates in Coordinator and Director Meetings as scheduled
Monitors documentation of services pro ided for compliance, quality and accuracy
Reviews all data collection as it relates to the PCP, including behavioral data collection
Completes discharge summaries for Day Services participants when applicable
Monitors and submits data collection related to Quality Enhancement Plans by the 16th of each month
Reviews and maintains record of Health and Safety Inspections and Emergency Response Drills, and develops task lists to correct any deficiencies
Submits and approves maintenance request forms and monitors completion of request to address and ensure health and safety
Investigates and reports participant-related incidents in accordance with agency policies and procedures and PORII Manages Petty Cash, as assigned, and in compliance with agency procedures
Participates in the agencys interviewing procedures as assigned
Orients and trains newly hired DSPs, Leaders and Managers as assigned
Ensures that DSPs, Leaders and Managers are trained, on the contents of PCP packets as they relate to the provision of Day Services prior to being assigned independent duties
Provides regularly scheduled assessments of assigned DSPs, Leaders and Managers skills in accordance with agency policies and procedures
Reviews and approves timecards and travel expense forms of assigned DSPs, Leaders and Managers
Monitors and ensures compliance with training requirements for assigned DSPs, Leaders and Managers
Investigates and documents need for disciplinary action or commendation of DSPs, Leaders and Managers
Reviews all Employee Communication Forms directed to assigned DSPs, Leaders and Managers and incorporates into formal supervision process
Facilitates and distributes minutes for Day Services Team Meetings
Participates in on-call rotation as assigned, and provides direct care supports as needed
Participates in the functions of agency committees and special projects as assigned
