The Admissions Coordinator interviews patients on admission, records required personal information, secure signatures, and prepare and supplies patient data to appropriate skilled nursing facility departments.The Admissions Coordinator is responsible for coordinating the daily referral and admission workflow of the skilled nursing facility. This position will serve to advise a prospective patient and family in the selection of healthcare and to facilitate the admission process in collaboration with the admission process. The position provides support for case management to the Interdisciplinary Team (Social Services Director, Nursing, and Therapy). This is facilitated through the development of positive working relationships with hospital discharge planners and other medical professionals.LPN/RN/Nursing background preferred.
Minimum Salary: 22 Maximum Salary: 27 Salary Unit: Hourly
