Providence Health Plan is calling a Clinical Care Coordinator who will: Be responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Physician Enterprise Conduct daily work that is inclusive of ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance Assure that an integrated approach to the patients' care coordination is fulfilled Serve as the liaison between Physician Services Enterprise providers, primary care clinics, specialist offices, patients, multiple Providence Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes Perform all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy Providence welcomes 100% remote work for applicants who reside within a 25-50 mile radius of the following office location: Washington: Renton Required qualifications for this position include: H.S. Diploma or GED equivalency 3+ years of experience as a medical assistant in a primary / specialty care setting Completion of an accredited Medical Assistant program and registered with Washington State Active Certification as a Medical Assistant through a nationally recognized program such as AAMA, AMT, RMA, CCMA or NCCT -OR- an Active Certification as a National Certified Pharmacy Technician registered with the State Board Preferred qualifications for this position include: Bachelor's Degree Why Join?
