JOB SUMMARY
This position is responsible for collection of the current home medication list of patients admitted to Blessing Hospital. This list of medications would include prescription medications and nonprescription (over the counter) medications currently administered to the patient in the non-hospital setting.
The Data Clerk will gather this medication information from:
Interview of the patient and/or significant other
The patient’s home medication list
Home medications in the possession of the patient
Long term care medication administration record
Physician clinic list
Retail pharmacies, as available.
The Data Clerk will document the list of current medications and work with the pharmacist to complete an initial review.
JOB QUALIFICATIONS
Education/Training/Experience:
REQUIRED:
High School Diploma or equivalent.
Completion of Pharmacy Technician Training Course.
Computer skills are required.
Excellent written and verbal communication skills.
PREFERRED:
Pharmacology education or pharmacy experience preferred.
Excellent typing skills preferred.
License/Certifications/Special Requirements:
Illinois registration as a Pharmacy Technician
Certification by the National Pharmacy Technician Certification Board or equivalent unless exempted by the state of Illinois due to Grandfathering
Pay Status:
