Social Services Assistant Summary
Assisting residents and families with the completion of admission paperwork. Directs residents and families to appropriate staff members to ensure that questions and/or concerns are addressed. Schedule IDT meetings for new admissions. Assist resident / families with long term care MaineCare application process as well as communicating with DHHS regarding status. Assist with tracking of Managed Care updates as well as assisting with the tracking of Maximus referrals. Performs clerical duties as needed.
Essential Job Functions:
Completion of admission paperwork for new residents.
Maintain and organize necessary forms for the admission process.
Perform clerical duties to assist the social work department as needed.
Assist with MaineCare applications.
Perform other duties as needed.
Essential Skills and Knowledge:
Strong written and verbal communication
Ability to maintain positive attitude and professional relationships with residents, families and staff
Long-term care experience preferred
