Atlantic Health System Supervisor, Social Work in Pompton Plains, New Jersey
Atlantic Health System is seeking a Full Time LCSW to workday shifts at the Chilton Medical Center.
Responsible for supervising and providing care to the patients by matching the patient's needs with appropriate services. Maintains and updates patient records and supervises and manages business operations and patient care.
Principal Accountabilities:
Maintains direct responsibility for educating, mentoring, and supervising Social Workers. Assess educational needs of dept. provides training and ongoing educational support. Provides guidance and supervision. Acts as resource to solve complex psychosocial, legal and financial problems of patients. Assures performance and regulatory standards are met. Collaborates with manager regarding need for staff disciplinary action. Assists in the evaluation, training, and counseling of employees.
Acts as a resource to the Social Work staff. Makes recommendations for growth, learning needs and assures that performance meets standards of practice. Provides skilled, concrete resource to staff. Attends MDR with staff. Initiates conferences to assure effective communication. Aware of LOC changes as determined by PA and assures appropriateness of discharge plan.
Acts as a resource to the manager in the daily management of the dept. Manages daily workflow of the social workers. Assures continuity and quality of service in collaboration with OMC care collaboration process. Responsible for key manager responsibilities in absence of manager. Collaborates with CM nursing coordinator. Assist administrative staff with problem solving.
Plans, develops the reporting of relevant data to support the Social Work program throughout the continuum. Reviews data to improve program, identify issues, trends that impact operational effectiveness. Assists in development of PI activities.
: 05. Assists in development of dept. Makes recommendations for growth, policy, and procedure development/changes. Serves as liaison with agencies to establish/maintain effective collaborative relationships. Maintains current professional knowledge, credentials, and skills.
Actively participates in PI activities. Facilitates scheduled meetings with staff to improve performance. Participates in analysis of care coordination, disposition delays in effort to improve quality, resource utilization and decrease LOS.
Collaborates with other depts in achieving goals of specific units/services. Attends and actively participates in all interdisciplinary forums as depts. representative. Demonstrates consistent capacity for positive interaction with other professionals through written and verbal communication. Serves as consultant to others. Provides information, teaching Assists with coordination of social work services within hospital and as well as outside agencies. Provides proactive support in order to achieve departmental goals and improve employee satisfaction.,
Assists to promote department efforts to be cost effective. Maintains established productivity levels. Remains informed of budgetary restraints.
Complies with hospital policy/procedures. Assist in developing and implementing departmental, patient, human resources and financial goals and objectives as well as requirements of TJC , NJDOH, and other applicable Federal, state, local and/or accrediting agencies. Utilize established chain of command to communicate problems.
Required Education and/or Experience:
LCSW with a minimum of 3 years with current licensure in the state of NJ Required.
MSW required
Relevant experience of Staff supervision to obtain LCSW, Managerial experience 2 years preferred, Acute Care hospital experience 2 years preferred, Managed Care experience, Electronic Medical Record, (EMR) experience preferred.
Relevant knowledge of community resources, insurance, legal, financial, and patient advocacy.
