Marshall-Starke Development Center, Inc. Direct Support Staff in Plymouth, Indiana

QUALIFICATIONS 1. Minimum of High School Diploma or equivalent. 2. No experience required. One year working with individuals with developmental disabilities preferred. 3. Current and valid Indiana drivers license and auto insurance coverage. 4. Job knowledge ability to communicate well orally and written. Ability to travel within the county and occasional travel maybe required out of the county area.

BASIC FUNCTION Responsible for implementation of Habilitation, Personal Assistance and Respite Services.

ESSENTIAL FUNCTIONS 1. Provide habilitation, personal assistance and respite services 2. Provide direct supervision, assistance or training for individuals assigned. 3. Documentation, training and assistance with Medication Administration. 4. Documentation, training, and assistance with incidental household maintenance. 5. Transportation, training, and assistance with access of community resources, shopping, medical appointments and related activities as defined in Plan of Care. 6. Provides recommended extension of therapy services, exercises and ambulation. 7. Implements Quality Assurance standards established by MSDC. 8. Documentation, training and assistance with personal grooming, bathing and dressing. 9. Completes incident reports and forwards to Supervisor. 10. Completes case notes on each individual assigned. 11. Completes data sheets on each individual assigned for habilitation services. 12. Communicates progress and changes to Supervisor. 13. Completes Service Logs for each individual assigned. 14. Assists with applications for benefits for individuals assigned. 15. Assists with money management procedures and managing residential living allowance supports for individuals assigned. 16. Completes required certifications for individuals assigned.

SECONDARY FUNCTIONS 1. Responsible for the health and safety of persons served at all times. 2. Expected to serve as role model for persons served. 3. Expected to serve as an advocate and to protect the human rights of the person served at all times. 4. Expected to participate in training as required by your supervisor or for all MSDC staff. 5. Other duties as assigned by your Supervisor.

CONTACT RESPONSIBILITY 1. Managers, direct support staff, persons served, family members, social service agencies and interdisciplinary Team members and visitors. 2. Position may require the employee to come in contact with body fluids. MSDC will provide training on blood borne pathogens and Hepatitis B vaccine to employees in this job classification.

ESSENTIAL FUNCTIONS FOR SITE COORDINATOR 1. Responsible to correctly assist and pay consumers bills on time 2. Responsible to appropriately and appropriately and accurately assist consumer with personal funds, payroll funds, banking within budget constraints. Will not cause checking account to be overdrawn or errors. 3. Responsible to order and keep adequate supply of medication for all consumers at work site and to notify Lead Staff when last refill is filled. Must have a minimum of 7 days of all medications at work sites at all times. 4. Responsible to notify in writing maintenance request when needed. 5. Responsible to notify in writing to supervisor if any employee needs further training, or staff issues. 6. Responsible to complete Incident Reports and notify supervisor in a timely manner 7. Responsible to involve consumer(s) in community integration (activities) on a weekly basis 8. Responsible to complete consumer grocery shopping by 10th of the month, and complete weekly menu with consumers, posted on refrigerator 9. Responsible to complete consumer LOA sheets accurately and completely on your shift 10. Responsible to keep Consumers IDs updated, not expired or incorrect address 11. Responsible to notify Supervisor if consumer family expresses concern in any area 12. Responsible to keep track of consumers birthdays, plan party. This would also include keeping dates for guardians and fami y members with consumer, sending cards or gifts if appropriate 13. Responsible to be appropriate at all times when communicating with families, guardians 14. Responsible to be appropriate when communicating concerns with supervisor 15. Responsible to efficiently communicate messages, phone calls, concerning welfare of consumer to supervisor

ENVIRONMENTAL CONDITIONS 1. Protection from weather conditions but not necessarily from temperature changes. 2. Ability to lift 50 lbs.
