Director of Social Services (Full Time)
AristaCare at Norwood Terrace is seeking a Director of Social Services to join our team. In this role, you will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient.
Develop and implement case management services to all residents.
Maintain contact with family (with resident’s permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
Maintain progress notes under federal and state guidelines for Long Term Care.
Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents’ individual needs.
Requirements:
LCSW or CSW is required. Must be a licensed social worker.
BA from an approved Social Work school is required.
Subacute and geriatric experience
Minimum one years’ experience with seniors. In Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
