THE UNIT:

AGH's team of Abdominal Transplant Coordinators serve as liaisons between patients, donors, and the multidisciplinary team to evaluate and coordinate for kidney, pancreas, and liver transplantation. In addition, the Abdominal Transplant Coordinators play an integral role in coordination of post-transplant care.

Job Summary: Responsible and accountable for a designated patient population for the purpose of providing care based upon enactment of the nursing process and floats to multiple areas within an assigned team based on skill level.

GENERAL OVERVIEW:

Coordinates and manages transplant patients' care throughout the entire transplant process.

ESSENTIAL RESPONSIBILITIES:

  • Coordinates all aspects of the transplant process including, but not limited to, referral, evaluation, waitlist, perioperative care, and follow-up care.

  • Performs assessment of patients' status, intervenes when appropriate and reports findings to transplant physicians.

  • Ensures that patient education is planned, developed, implemented, evaluated and documented in patient chart and transplant database. Coordinates and presents formal in-service education programs for professional, as well as, general audiences.

  • Coordinates the donor/recipient process as a liaison with the Organ Procurement Organization (OPO), referring physicians, histocompatibility laboratory, transplant physicians, hospital staff and patients.

QUALIFICATIONS:

Minimum

  • 3 years clinical RN experience

  • Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).

  • BSN

  • CCTC required within three (3) years of start date

  • CPR – American Heart Association

  • Act 34 Criminal Background Clearance Certificate

  • Act 33 Child Abuse Clearance Certificate

  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Preferred

  • 1 year transplant experience

  • American Board of Transplant Coordination Clinical Certification Examination
