Position type: Full-time, Hybrid work arrangement: up to 2 days a week remote (after introductory period)!

Hourly: $30.00/hour

New hire bonus: $2,000!

Location: Achieva Office (Southside)

 

Function:

Provides indirect and direct clinical support to people with disabilities throughout Achieva Support. Responsible for providing oversight to clinical team members, including LPNs and other clinical staff as assigned to ensure medical supports are provided to people within multiple Achieva Support programs. Responsible for assessing and reporting clinical needs as needed, educate staff and people supported on medical care requirements, and health and wellness improvements. Act as a liaison to internal and external healthcare professionals to promote health and safety to people supported. Assist with medical appointments as needed.

 

Responsibilities:

  • Support the achievement of every person's personal outcomes/goals by promoting good health practices and advocating accurate medical and psychiatric care.
  • Follow the Achieva and Achieva Support mission, and values.
  • Provide clinical nursing assessments; mentorship and consultation for medical situations involving people supported throughout Achieva, to ensure their medical and mental health needs are addressed and met.
  • Communicate external/contracted healthcare practitioner recommendations to applicable Achieva Support employees; ensuring their understanding and adherence to all prescribed medical orders, facility discharge instructions, and/or health promotions outlined in the person's plan of care and/or health screening tools.
  • Collaborate with program Vice Presidents and/or their management team on clinical recommendations made for persons supported; as well as, identifying any clinical and mental health needs relating to their respective programs.
  • Provide clinical instruction/training to Achieva Support employees as needed.
  • Consult and advise on the development of clinical policies and procedures and/or training for the agency.
  • Respond to Achieva Support employees by phone or on-site in the event of a crisis situation and/or medical emergency, and ensure that the appropriate parties, both professionals and family, have been advised of any clinical occurrences.
  • Participate in a clinical on-call rotation attending to calls from employees and/or management that occur after regular business hours and require clinical guidance/instruction.
  • Coordinate hospitalizations, admissions/discharges, and/or transfers to skilled aftercare facilities and maintains daily contact w/ hospital personnel and/or skilled nursing facility (SNF) staff, to acquire clinical updates on persons supported by Achieva. Discuss therapy progress; advise hospital/SNF personnel of what person needs to return safely to their home. Coordinate all discharge needs as applicable and ensure that all medications that are ordered are received by the pharmacy and scheduled for delivery to the site.
  • Attend team meetings for people supported, when requested.
  • Serve as contact between the pharmacy and Achieva staff. Assist with the obtaining of new medication orders and discontinuing previous medication orders; performs re-orders when requested, and medication review when required of pharmacy-generated electronic medication records and forms.
  • Participate in state nursing network associations; and follow state health care initiatives for the best processes in the clinical and IDD systems.
  • Participate in Achieva Support's Incident Management process as needed and acquire certification as an abuse investigator.
  • Participate in any applicable program licensure inspections.
  • Participate in the completion of the HRST process providing clinical reviews once all required training is completed.
  • Maintain regular contact with the Director of Clinical Services to provide updates regarding current clinical concerns/needs of all Achieva programs; and collaborate for a plan of resolution of these clinical concerns.

Qualifications:

Satisfactory completion of a registered nursing program. Valid Pennsylvania RN license required. Bachelor's Degree or equivalent experience in Nursing preferred. Experience supporting people with intellectual developmental disabilities in long-term services and supports preferred. Computer literacy required. Excellent organizational and communication skills. Some local travel and some evenings and weekends may be required. Satisfactory Act 33/34 and FBI clearances are required. A vehicle available for daily use required.
