UPMC Intake Coordinator - Transplant (Center for Liver Care) in Pittsburgh, Pennsylvania

The Center for Liver Care is seeking an intake coordinator to join our team, where they will play a vital role in patient access, scheduling office visits, and coordinating procedures. The ideal candidate will be highly organized and detail-oriented, ensuring accuracy and effective follow-up. Top applicants will also have strong interpersonal skills, excellent verbal and written communication abilities, and a talent for collaboration. This full-time position is based in the Kaufmann Medical Building and operates Monday through Friday during daylight hours.

Responsibilities:

  • Manage initial intakes of referred patients referred to the UPMC Center for Liver Care (CLiC).

  • Asist with the coordination of care for patients with liver disease, in collaboration with the medical team.

  • Manage referral source missing information.

  • Participate in multidisciplinary care of patients with liver disease with nurses, advances practice providers, physicians, and other medical personnel.

  • Obtain basic medical histories and personal information of new patients.

  • Update medical records of new and existing patients.

  • Review documents for accuracy and completeness.

  • Maintain accurate documentation.

  • Enter data into the electronic medical record.

  • Maintain weekly/monthly productivity.

  • Collects initial referral information via telephone, fax, or e-mail and distributes information to appropriate team member in a timely and efficient manner.

  • Demonstrate the ability to establish priorities, perform assignments and meet productivity thresholds in the allotted time frames while maintaining accuracy and attention to detail.

  • Support Transplant Coordinators and other members of the Transplant Team.

  • Maintain patient confidentiality.

  • Coordinates and manages living donor evaluation including but not limited to scheduling all required appointments and testing, obtaining authorizations, staffing donor clinic and preparation for donor selection committee meeting.

  • General data entry responsibilities to include the collection and verification of demographics and insurance information, test results, and other pertinent information into and EPIC databases.

  • Prepare, organize and maintain confidential patient charts and clinical records.

  • High school diploma or equivalent required.

  • Associate degree preferred.

  • Two years' experience with administrative support in an outpatient clinic, physician office or other health care setting.

  • Knowledge of medical terminology required.

  • Ability to handle a high-volume workload and to remain focused and organized under pressure.

  • Ability to multi-task and meet deadlines.

  • Ability to perform duties with significant degree of independence and initiative.

  • Strong interpersonal, communication, and customer service skills required.
