Trinity Health Housing Stabilization Coordinator in Pittsburgh, Pennsylvania

Description:

POSITION PURPOSE:

The Housing Stabilization Coordinator provides a multitude of services within the Emergency Shelter to include intakes and assessments, complete and/or update individual goal plans; provide necessary support for access to items to maintain daily living while an individual served is engaged at Emergency Shelter; monitor and deploy de-escalation techniques to maintain a safe and healthy environment for all individuals served; assists individuals served with all necessary documentation for moving towards a housing first model of operations and support.

WHAT YOU WILL DO:

  • Ensures Procedures and Policies are implemented and effectively followed

  • Ensure achievement of benchmarks for performance are met

  • Understands and appreciates the demands of the population served in Emergency Shelter

  • Ability to troubleshoot questions related to program guidelines

  • Collaborate with colleagues and develop recommendations to present to Supervisor to add (or decrease) services based upon individuals served needs

  • Timely completion of required tasks per Emergency Shelter shift checklist

MINIMUM QUALILFICATIONS: (must match requirements in job description)

  • Bachelor’s Degree in Human Services or a HS Diploma OR Equivalent with 4 years’ experience.

  • Act 33/34 and 73 Clearances

This is a non-exempt (hourly) position. 3 pm to 11pm Tuesday to Saturday OR Sunday to Thursday.
