DESCRIPTION OF WORK

As an Aging Care Manager 2, you will conduct intake casework for the Community Based/Entry Division, Information/Assistance/Intake Unit, while working under the general supervision of the Casework Supervisor.  Within this role, you will conduct the following duties:

Meets AIRS (Alliance of Information and Referral Systems) standards for certification as a CIRS-A (Aging) specialist as eligible and testing is available.  Accumulates necessary credits to maintain certification.

Answers calls and meets with walk-ins (in-person or virtual) to DHS/AAA SeniorLine Call Center daily and as scheduled.  You will also explain the Intake/Assessment/Care Management process for relevant programs to callers and walk-ins.

Identifies and confirms needs and provides callers and walk-ins with relevant accurate information and assistance to access appropriate resources and programs to meet those needs using the Options/Person Centered Counseling approach.  Conducts the intake interview while completing intakes for in-home services, whether referred via SeniorLine phone system, the SeniorLine email, fax, a walk-in or from other sources.

Requests any printed information, brochures, and applications from unit clerical staff to be sent to participants in a timely manner.  Refers those with computer access to DHS/AAA website for this relevant information, as appropriate.

Investigates Dialtrac, County, and DHS/AAA requests and complaints as assigned.  Submits findings to Caseworker 3 or Casework Supervisor for review when completed.

Completes Reports of Need (RON) for consumer situations described as having the potential to meet Protective Services requirements.

Reviews RON with Caseworker 3 or Casework Supervisor immediately upon completion.

Refers RON to Protective Services for further investigation.

Documents all work in the appropriate computer databases in a timely manner.

Meets with older adults at Community Resource/Info/Health Fairs to discuss DHS/AAA services and programs.

Assists with community education including reviewing and developing material for the DHS website as well as electronic and hard copy publications on DHS/AAA services and programs, as assigned.

Assists in researching potential participant resources.  Submits information to unit Casework Supervisors for possible inclusion in DHS/AAA website.

Participates in on-going training to develop and enhance job knowledge and skills.

Work Schedule and Additional Information:

Full-time employment

Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.

Salary:In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS

Minimum Experience and Training Requirements:

Six months as an Aging Care Manager 1 or a County Caseworker 1; or

Successful completion of the County Social Casework Intern program; or

Six months of professional human services or nursing experience; and a bachelor's degree in s cial sciences, behavioral sciences, human services, or a closely related field; or

An equivalent combination of experience and training.
