Please note work hours may fluctuate depending on the Participant and business needs, flexibility is required.Job Summary:At Mountain View Pace, our Master Social Workers are responsible for the direct social work case management services to Mountain View Pace Participants. Provides social services support, including Participant screening, case management, counseling, and referral.Duties/Responsibilities:Participate in the Interdisciplinary Team’s initial assessments, care planning, and ongoing re-evaluations of the Participant’s case.Maintain current written case management records, including periodic reassessment of program Participants.Provide individual and family counseling; develop and lead group counseling and activities.Refer Participants to appropriate community agencies or facilities, acts as a liaison with such organizations and as an advocate for Participants.Create effective relationships among staff geared to building and maintaining a cohesive team.Consult with and advise staff members on the relationship of social, emotional, and cultural factors to health and medical care and the availability of social services in the community.Participate in program and policy development of the social work component of the program.Communicate Participant changes to team members.Participate in on-going Quality Initiative training as directed.Other duties as assigned.Required Skills/Abilities:Must possess the ability to make independent decisions when circumstances warrant such action.Must be able to deal tactfully with personnel, Participants, family members, visitors, government agencies/personnel, and the public.Ability to organize work and follow through on assigned tasks.Excellent written and verbal communication skills.Must have the cognitive ability to process and perform tasks for Participant safety.Attends and participates in continuing educational programs.CPR and First Aid Certification.Education and Experience:Master’s Degree from an accredited School of Social Work (MSW) with one year of experience working with the frail/elderly population.One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.This position requires a valid state driver’s license and reliable transportation.Requirements:Requires valid State driver’s license with a clean driving record.Successful completion of background check, physical, drug screening, TB ScreeningPhysical Requirements:Works inside the facility throughout the Participant and office areas.Sits, stands, bends, lifts, and move intermittently during working hours.Is subject to frequent interruptions.Must be alert for Participant’s safety.Is subject to lifting, carrying, and supporting Participants.Our organizations provide equal employment opportunities to all employees and applicants for employment.
