Description
Position Summary:The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.Essential Job Functions:Provides case management services and assistance to program participants strugglingwith substance abuse, and those in recovery;Assesses the needs, conditions, and desires of assigned program participants;Educates and refers program participants to community agencies and businesses thatappropriately address their needs for food, shelter, clothing, employment,transportation, etc.Assists participants with establishing or re-establishing community support systems;Acts as advocate on behalf of program participants to improve access to vital services;Participates in clinical staffing decisions regarding the participants current strengthsand resources, and their need for additional resources;Completes documentation of services provided in a timely manner;Provides input into the patients treatment needs and disposition;Conducts assessments of program participants and provides therapy as needed;Reviews and documents all clinical services and activities;Participates in case conferences involving assigned program participants;Prepares Discharge Summaries, as needed;Assists other WestCare staff as needed to promote cooperation and portray aprofessional image to the community;Embraces and embodies the mission, vision, guiding principles, clinical vision and goalsof WestCare Foundation;Performs any other duties as assigned.
Qualifications
Essential Qualifications:Certifications/Licensures:CADC or similar certification preferred; Preference will be given certified candidates;Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required.Education:Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred;High School Diploma or equivalent is required;Experience and Competencies:Experience working with individuals suffering from mental health and substance abuse disorders required;At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred;Ability to define problems, collect data, establish facts, and draw valid conclusions required;Excellent communication skills are required;Ability to exercise good judgment and discretion required;Ability to work well in a team environment required ;Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required;Professional appearance and demeanor required;Must be culturally/linguistically sensitive to populations served;Intake and assessment experience is highly preferred;Bilingual ability is preferred;Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired;
