JFCS Administration Youth & Family Specialist - Part Time (Phoenix) in Phoenix, Arizona
Perform out of office care coordination; facilitate CFT meetings; and provide home- or community-based therapeutic and supportive services in accordance with agency procedures. Perform administrative tasks in accordance with agency procedures.
Provide in-home and community based intervention and education to support consumers and their families.
Serve as facilitator of Child and Family Team (CFT) meetings while assisting families in understanding the CFT process.
Completes all CFT tasks to support the development of successful meetings including outreach and engagement efforts.
Completes documentation in a timely manner.
Performs on-going case coordination services according to CFT meetings and Behavioral Health Service Plan.
Ensures coordination of Behavioral Health Service with third party agencies such as Child Protective Services, Probation and Parole Departments, Division of Developmental Disabilities, and other providers.
Additional duties as assigned.
Must have a Bachelor's Degree in a behavioral health related field; or a Bachelor's in any discipline with at least one year full-time direct behavioral health work experience; or an Associates Degree in behavioral health related field with at least 4 years of full-time direct behavioral health work experience.
Must have a DPS Level 1 Fingerprint Clearance Card without restricion(s)
Must have valid AZ auto insurance
Must have reliable transportation
Bilingual in Spanish preferred
