The Speech Therapist consults, evaluates, plans and administers skilled Speech Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.

Requirements include :

1 year of clinical practice as a licensed Speech Therapist required.

Currently licensed to practice as a Speech Therapist in Pennsylvania

Home care or acute care experience required

Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.

Currently Valid Driver’s license in the state of PA

CPR certification.

Regular availability to work.

Recent home care experience preferred.
