Description:

Nazareth Hospital, a member of Trinity Health Mid-Atlantic, is currently seeking a Full - Time Registered Nurse for our Outpatient Cardiopulmonary Rehab program. The position requires a bachelor's degree in nursing and a current, valid PA RN license, BLS and ACLS certifications.

Qualifications also include:

• Previous IP/OP cardiopulmonary rehab experience (preferred).

• Previous telemetry or critical care experience (preferred).

• Certification in cardiac rehab (preferred).

• Strong knowledge of cardiac rhythms and arrhythmia recognition.

• Strong knowledge of pulmonary diseases, including COPD and interstitial lung disease.

• Critical thinking and judgment skills with the ability to evaluate patient response to exercise during testing/intervention.

• Ability to multitask and work well under stress and in an emergency situation.

• Ability to work interdependently.

• Ability to develop an individualized exercise prescription for cardiac, pulmonary, and vascular patients based on clinical results of assessment and/or exercise stress test.

• Ability to evaluate the patient's responses to exercise conditioning.

• Strong interpersonal and educational skills required for patient and family education.

• Demonstrates strong knowledge of cardiac risk factors and lifestyle modifications.

Schedule: Full-time, 9-hour day shift (Monday through Thursday)

Responsibilities include:

• Evaluate new patients and interpret findings.

• Formulates an individualized exercise program for both low level and high sessions according to established approved protocols.

• Assess risk factors that contribute to cardiac diseases; encourage patient to be an active participant in his/her health care and help patient with modification plan to include diabetes management, HTN care, diet, exercise, depression, stress and smoking cessation.

• Utilizes motivational strategies to promote compliance.

• Implements cardiac teaching.

• Monitor BP, heart rate and rhythm, blood glucose, and pulse ox, as applicable, before, during, and after each exercise sessions

• Extends psychological support to patient and family.

• Documents responses to exercise and other rehab care with each session.

• Utilizes research findings to maintain an up-to-date program.

• Functions as a resource person and consultant to other health team members, problem solving, teaching, and recruitment of inpatients and outpatient Cardiopulmonary Rehab patients.

• Implements a corrective action plan as a result of the Quality Review findings.

• Evaluates and documents goal attainment and patient response to nursing interventions with each exercise session.

• Alters the plan of care as indicated by evaluation.

• Provides progress reports to the physician on evaluation of patient’s progress.

• Provide education on cardiac rehab to inpatients after a heart attack, heart failure, or stent placement prior to their discharge. Provide education to colleagues and providers on benefits

• Review/Revise program policies, procedures and protocols to recommend changes to improve/update the program.

• Conducts quarterly QA reports to evaluate the safety and effectiveness of the program.

• Conducts discharge interview as subjective program evaluation.

• Assists with the registration and scheduling of patients through the EHR
