Job Title:Health Home Plus Care Manager
Location:Finger Lakes Region, NY

Job Summary:
TheHealth Home Plus Care Managerprovides intensive care coordination services to individuals with Serious Mental Illness (SMI). This role involves promoting independence and preparing clients for step-down into regular care management. Through assessments, crisis planning, and person-centered care, the Care Manager ensures clients understand and navigate medical, behavioral, and social service systems. Collaboration with families, providers, and community partners is essential to achieving positive health outcomes.
Required Qualifications:
Education:
Bachelor’s degree in a qualifying field, OR
NYS teacher’s certificate requiring a bachelor’s degree, OR
NYS licensure and registration as a Registered Nurse with a bachelor’s degree, OR
Bachelor’s degree (or higher) in any field with five years of experience working directly with individuals with behavioral health diagnoses, OR
Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
Experience:
Two years in one of the following areas:
Providing direct services to individuals with Serious Mental Illness (SMI), developmental disabilities, or substance use disorders.
Linking individuals with SMI, developmental disabilities, or substance use disorders to a wide range of community services (e.g., medical, psychiatric, social, educational, legal, housing, financial).
A master’s degree in a qualifying field may substitute for one year of experience.
Licensure/Certifications:
Valid NYS Driver’s License with a safe driving record.
Availability of a personal vehicle for work-related travel (including transporting clients).
Job Requirements:
Key Responsibilities:
Conduct comprehensive assessments, outreach, engagement, service linkages, and treatment coordination using evidence-based practices.
Perform screenings or arrange detailed assessments for risk factors such as substance use, mental health crises, harm to self/others, abuse, or domestic violence.
Plan and coordinate care for high-need SMI individuals, including:
Navigating mental health systems and making referrals (e.g., mental health housing, crisis intervention, peer support).
Collaborating with inpatient staff and Managed Care Organizations (MCOs) to ensure smooth transitions from inpatient care.
Completing plans of care for HARP members and coordinating Home and Community-Based Services (HCBS).
Maintain engagement with individuals who may be resistant to care, have difficulty adhering to treatment plans, or have histories of homelessness, justice involvement, or first-episode psychosis.
Employ engagement techniques such as:
Motivational Interviewing.
Suicide Prevention and Risk Screening.
Trauma-Informed Care.
Recovery-Oriented Approaches (e.g., WRAP).
Utilize technology, including Telehealth, to enhance communication among peers, providers, and community agencies.
Monitor and report on clinical indicators, performance measures, patient satisfaction, and adherence to care plans.
Participate in on-call duties for nights and weekends as assigned.
Skills & Competencies:
Effective communication and collaboration with culturally diverse individuals, staff, and service providers.
Knowledge of local behavioral health services and substance use treatment options.
Ability to manage multiple tasks in a fast-paced environment with strong problem-solving and crisis intervention skills.
Proficient in typing (30 WPM minimum) and skilled in Microsoft Office (Word, Excel, Outlook) and Internet navigation.
Experience using electronic health records (EHRs).
