Providence St. Joseph Health Care Management Assistant in Olympia, Washington

Description Care Management Assistant - Providence St. Peter Hospital - Olympia, WA SCHEDULE: 1.0 FTE, averaging 40 hours per week. 8-hour shifts, 8a-5p PST. Mandatory weekend rotation. Under the direction of the Care Management leadership team, the Care Management Assistant (CMA) will work collaboratively with the Social Worker Care Managers (CM-SW), RN Care Managers (CM-RN), Utilization Review RN's (UR-RN) and/or other business partners by performing multiple support service duties that facilitate the seamless transition of patients throughout the care continuum using current knowledge of inpatient and outpatient processes and community services. The CMA position requires frequent contact with patients and families and assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with medical staff, business office, medical records and other hospital staff. This position requires the individual to take initiative and be proficient in prioritization skills, organizational skills, communication skills and a commitment to quality and excellence in service. Required Qualification: One (1) year job related experience in a healthcare setting. Preferred Qualifications: Associate's Degree. Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire. Prior experience in discharge planning, billing, utilization review, financial counseling or registration. Proficiency in medical terminology. Why Join Providence?
