Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties.

WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health, and senior service programs.

Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups, including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions.

As a Certified Case Manager for our Adult Mental Health Team, you will work with adults who need community resources. You will need to have experience working with mental illness and be willing to work in the community, schools, hospitals, residences, shelters, etc. You must have strong collaboration and communication skills and be willing to work a flexible schedule if needed.

We will provide training to become a Certified Case Manager. Spanish speaking is a plus, and a stipend will be available upon passing the Spanish competency test. You will be driving and need a valid Utah Driver's License.

i. TCM: Provide monitoring, linking, coordinating, advocating, outreach and other medically necessary services to clients. Coordinate with Adult Team members and other community service providers to enhance quality of life and avoid illness and/or hospitalizations exacerbations. ii. Teach daily living skills with focus on recovery and increasing client's ability to care for self in the least restrictive environment by individual and/or group home development. iii. Focus on Critical Time Intervention and coordination with the inpatient unit and treatment team to help maintain client's stability outpatient. iv. Documenting services accurately, professionally and within treatment goals of primary service coordinator as mandated by WHS. v. Provide other medically necessary services to non-TCM clients as the adult team staff determined necessary. vi. Other duties as assigned.

a. Minimum of high school diploma/GED. b. Social Service Worker License preferred. c. Certified Case Manager within 1 month of hire date and renew every 3 years. d. Valid Utah Driver's License. Defensive Driving and clearance to operate WHS vehicles.

 

Must be willing to complete and criminal background check and pre-employment drug screening.
