Charter Care Health Partners Social Worker, BH Inpatient, Lead in North Providence, Rhode Island

Summary: Under the Direction of the System Manager, the Lead BH Inpatient Social Worker is responsible for providing clinical supervision and support to master’s level clinicians and interns on the LICSW track while ensuring compliance with regulations. Utilizes clinical skills to assess patient and family needs for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Performs clinical duties including comprehensive assessments and evaluations, individual treatment and family meetings for every patient. Responsible for digital documentation and tracking of treatment plans and plan reviews, clinical documentation and groups, as needed or requested. Works in collaboration with other specialties on the unit. Performs functions related to Patient Advocacy; Utilization Review; Resource Management; Continuum of Care Management, Discharge planning and Clinical Documentation Management. Consistent demonstration of leadership and mentorship.

Responsibilities: The Lead BH Inpatient Social Worker provides individual and group supervision ensuring that high quality clinical services are being provided. The Lead audits patient records ensuring that clinical records are accurate and up to date. The Lead provides reeducation to staff whose documentation is not accurate or timely. The Lead provides clinical oversight of treatment planning and crisis management. The Lead participates in chart audits and special projects to meet regulatory standards. The Lead will carry a case load in the department they support and will meet the expectations of the Clinician job descriptions.

The inpatient social workers will report directly to the incumbent. The incumbent will develop business planning and programming that supports the goals of the Triple Aim. Performs functions to prevent financial loss to the hospital while ensuring high quality of care for all patients.

Education: Master Degree in Social Work

Experience: Minimum of 5 years clinical experience in acute behavioral health. Utilization review and inpatient behavioral health experience preferred. Computer database management experience and case management experience preferred.
