State of OK JD McCarty Center Case Manager I - C.A.R.E Program in Norman, Oklahoma

The J.D. McCarty Center is a fun, energetic place that is always seeking new and improved treatment strategies to help our patients and their caregivers.  At the J.D. McCarty Center, we serve children 0-21 years of age in a variety of settings including inpatient, outpatient and school settings.  

The J.D. McCarty Center's C.A.R.E. Program is looking for a Case Manager to work with patients and their caregivers to determine their needs and goals for the patient's continuous care and growth.  This person will connect the patient and/or caregivers with appropriate support services/resources that are available at J.D. McCarty Center and their community as well as throughout the state. 

Salary starting at: $42,500 per year plus PTO, Insurance, and Retirement benefits.

No On-Call or Weekend hours.

This position has some work-related travel responsibilities which will require the person to possess a valid Oklahoma driver’s license.  This person will need a bachelor's  degree in a human services field and preference will be given to candidates with a BSW or MSW.  This person must have a working knowledge of Microsoft Office Suite, Office 365, and Zoom. This position works with the patient's caregiver and does not include direct patient care.  

Responsibilities include:

  • A ssists with connecting patient and/or family/caregivers with appropriate support services/resources that are available at J.D. McCarty Center and throughout the state. Advises family/caregivers of available supports, notifies of application process, and assists with connecting families/caregivers to services statewide and within the family/caregivers’ community. Consults with JDMC staff as needed.

  • Assesses training and development needs for families, caregivers, schools, or other service providers related to understating ID/DD and behavioral support needs and refers them to the CARE Team Clinician for further training needs.

  • Completes and maintains current documentation within the time frame and guidelines specified by the agency and departmental policy including progress notes, staffing reports, admission and discharge summaries, and billing statements.

  • Coordinates outreach services with CARE team members and other Center staff as needed for training and other support services in the home, school, or other community settings. Attends outreach appointments and facilitates services between treatment team, outside resources, and family/caregivers.

  • Serves on various agency, community, and state committees which are directed towards the achievement of department and agency goals, missions, and visions.

  • Accomplishes patient’s care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; and working with interagency collaborations

  • Works to achieve caregiver/patient goals by completing intake interviews; determining need for therapeutic medical, psychosocial, and psychiatric evaluations; and maintaining treatment objectives and plans.

  • Establishes treatment plans by setting expectations with the family, coordinating services, arranging resources, assisting with service applications, and completing referrals for services at JDMC, statewide, and within the family/caregivers’ community.

  • Monitors case status by verifying caregiver and patient participation, observing and evaluation treatment responses, advocating for needed services and supports, obtaining additional resources, intervening in crises, and assisting with developing crisis plans, and providing personal support.

  • Communicates client progress by attending interdisciplinary meetings, communicating case status with the treatment team, and identifying barriers to treatment success.

  • Prepares client discharges by reviewing and altering discharge plans as needed, coordinating discharge requirements, outlining goals achieved, and providing resources.

  • Fields intake calls and connects families and caregivers with appropriate referrals. Monitors for new CARE referrals and completes follow up with guardian and/or Child Welfare Worker. Assists with maintaining proper documentation of new referrals.

  • Assists with maintaining and assessing QA measures.

  • Completes various weekly and monthly reports and presents them to CARE team.

  • Processes internal referrals to JDMC services, such as outpatient therapies and clinics. 

  • Completes other duties as assigned.

    Preferred Education/Experience:

    Education and Experience requirements at this level consist of a bachelor’s degree in a human services field* and one (1) year of experience working directly with individuals with intellectual and/or developmental disabilities; or possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities.*For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.   A bachelor's degree in Social Work is preferred.  This position may have responsibilities that require work-related travel.  Preference will be given to bilingual candidates.  Must have working knowledge of Microsoft Office Suite and O365.

    About J.D. McCarty Center:

    At J.D. McCarty Center patients are admitted for a 30-day evaluation during which they are assigned a treatment team consisting of nursing unit, physical therapy, occupational therapy, speech therapy, nutritional services, social services, psychological services, behavioral analysis, and recreational therapy. Our 80-acre campus includes an on-site elementary, intermediate, and high school which is staffed by Norman Public School teachers. Patients may continue treatment needs beyond the 30-days as an in-patient in habilitation status based upon progress or placement needs. We are a tobacco and vape free facility.
