Actalent Licensed Practical Nurse Care Coordinator in Norfolk, Virginia

Job Title: Licensed Practical Nurse - Care Coordinator

Job Description

Prepare patients for visits including assessing chief complaints, reconciling medication lists and allergies, and assessing smoking status. You will also be responsible for preparing exam rooms, restocking clinical areas, sterilizing instruments, and maintaining medical equipment. Document all interventions in the electronic medical record, ensuring chart completeness, and handling test results and billing. You will manage patient messages, prescription refills, lab results, referrals, and more while maintaining strict adherence to confidentiality and safety protocols. Additionally, collect patient satisfaction surveys and maintain contact with patients to assess medical status and medication adherence.

Hard Skills

  • Practical Nurse License

  • Ambulatory and Diabetes/Endocrinology experience preferred

  • BLS certification

Soft Skills

  • Documentation skills

  • Communication skills

  • Organizational skills

Work Environment

  • Outpatient office environment with a fixed schedule of Monday to Friday, 8 AM to 5 PM, with an hour lunch break.

  • No weekends or holidays are required.
