Location: Newtown Square Branch Office (Serving Greater Delaware County & West Philadelphia Areas)

As a Home Care RN (Mom/Baby) , you'll oversee postpartum care and provide education to new mothers and their newborns in accordance with physician orders and our organization’s policies. You will:

  • Conduct initial assessments and develop personalized care plans in collaboration with our Population Health Case Manager.

  • Provide functional support to Home Healthcare Aides and LPNs as needed.

  • Deliver therapeutic interventions to ensure top-level client outcomes and patient satisfaction.

Minimum Qualifications:

  • Graduate of an accredited nursing education program

  • Licensure as a Registered Nurse (RN) in Pennsylvania

  • At least one year of experience in maternal/newborn care in a hospital setting

  • Valid driver’s license and reliable transportation
