Location: Newtown Square Branch Office (Serving Greater Delaware County & West Philadelphia Areas)

As a Home Care RN (Mom/Baby) , you'll oversee postpartum care and provide education to new mothers and their newborns in accordance with physician orders and our organization’s policies. You will:
Conduct initial assessments and develop personalized care plans in collaboration with our Population Health Case Manager.
Provide functional support to Home Healthcare Aides and LPNs as needed.
Deliver therapeutic interventions to ensure top-level client outcomes and patient satisfaction.

Minimum Qualifications:
Graduate of an accredited nursing education program
Licensure as a Registered Nurse (RN) in Pennsylvania
At least one year of experience in maternal/newborn care in a hospital setting
Valid driver’s license and reliable transportation
