Position Summary

Lincoln County's Maternal, Child & Family Health team are looking to add a bilingual case manager to their team to better serve our non-English speaking community members.

We are looking to fill either a Case Manager 1 or a Case Manager 2 position.

Case Manager 1: $22.64 - $30.43 Hourly

Case Manager 2: $26.41 - $35.49 Hourly

The person in this position will provide case management level services to clients involved in Maternal, Child & Family Health (MCFH) Home Visiting Programs. Serve as a front-line public health worker who acts as a liaison, link, intermediary and navigator between health and community resources to facilitate access and to improve the quality and cultural competence of service delivery. In this division you will be expected to work alongside the home visit nurses and Community Health Workers (CHWs) to provide home visiting activities and targeted case management (TCM) in accordance with the Babies First! And CaCoon program models. Act as a lead for CHWs in the MCFH programs.

This role is also responsible for building individual and community capacity in increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, navigation, care coordination, social support and advocacy. Working as part of an interdisciplinary team in Public Health’s Maternal, Child and Family Health program. Assisting in training and mentoring CHWs that join the team.

Essential Functions/Major Responsibilities

Perform case management duties for MCFH clients under direct supervision of a Registered Nurse (RN), including:

  • Assist in conducting intake screens and assessment of individual case management and client needs. Use Oregon Mothers Care to collect screening and intake data.

  • Provide care coordination, advocacy& community referrals.

  • Apply theories and principles integral to implementation of the Babies First! and CaCoon home visiting models.

  • Provide targeted case management services to families in accordance with the Oregon Administrative Rules.

  • Advocate for individuals and communities.

  • Work independently or with a registered Nurse in clinics, family’s home, or other locations in the community.

  • Set up schedules, track clients who are often difficult to find, and perform visits on a drop-in basis if needed.

  • Provide coaching and social support by motivating and encouraging people to obtain care and utilize other community resources.

  • Document all client interactions in the EHR and collect data for evaluation purposes in accordance with established procedures.

    Provide cultural mediation among individuals, communities and health and social service systems

  • Teach communities about how to use health and social service systems.

  • Provide culturally appropriate health education and information that matches linguistic and cultural needs of participants and communities.

  • Provide direct service such as: basic screening tests, translation and interpretation, medical and social history, social determinants of health, program eligibility and barriers to care.

Team Lead for MCFH Community Health Workers

  • Assist with training and mentoring new CHW’s in the MCFH programs

  • Take part in annual Title 5 project planning and implementation.

    Other functions:

  • Interact with community partners and resources as necessary to assure population needs are being met.

  • participate in team meetings, trainings, and activities to further the health of the community.

  • Perform administrative support duties.

  • Maintain division productivity level standard as established for this position.

  • Meet weekly caseload requirements.

Typical Qualifications

Minimum Education Level

CM1: High School Diploma / GED

CM2: Bachelor's in Public Health or related field

  • Experience can be used in lieu of minimum education requirements. Will accept Associates with 2-3 years of experience in maternal, child and family health and case management; or a High School diploma/GED with 5 or more years of experience in maternal, child and family health.

Minimum Experience Level

CM1: Minimum of two years’ experience in maternal, child and family health and case management.

CM1/CM2:

  • Bilingual and/or bicultural competencies for the Latino population are required.

  • Qualified or certified interpreter required.

  • Community Health Worker Certification or equivalent at time of hire.

  • CPR Certification within 30 days of hire.

Driver’s License

Oregon Driver's License required within 30 days of hire.

Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
