Title: Senior Case Manager

Program:Parole Support Treatment Program.

Salary:$52,000

Position Overview:

Under the general direction of the Director of Forensic Services and the Case Management Team Leader, the Senior Case Manager is responsible for maintaining a caseload as well as directing a staff of Intensive and Supported Case Managers in providing required services and support to the clients to enable them to stabilize themselves and ultimately live independently in the community; performs related work as assigned by the Case Management Team Leader.

Essential Duties & Responsibilities:

The Senior Caseworker maintains a caseload of 6 to 7 clients as well as directing staff in the following activities:

  • Provides guidance and direction to Intensive Case Managers and Supported Case Managers engaged in assessing clients, getting them entitlements and benefits, assisting them in developing their service plans which enable them to stabilize themselves and live independently in the community.

  • Ensures that Case Managers adhere to established time frames for engaging the clients—conducting an initial interview within 7 days of admittance into the Program, conducting an assessment within 15 days of the initial contact, and creating an Individual Case Management Plan for each client within 30 days of the assessment.

  • Ensures that Intensive Case Managers are visiting their clients at least 4 times per month and that Supported Case Managers are visiting at least 2 times per month.

  • Responds directly or makes sure that staff is responding to all requests or pleas from clients.

  • Monitors clients to ensure that they are doing well in managing their affairs—attending programs, staying on their medication plan, managing their budget, not showing signs of relapse, and taking action to move to the next level of their plan. Initiates corrective action if any deficiencies are observed or reported by the Housing Counselors.

  • Attends weekly meetings with the Director, Housing Counselors, and other staff on the status of all clients in the Program.

  • As needed, liaisons with Housing Counselors to make sure that the clients’ needs are being met.

  • Monitors staff to make sure that required data and case notes are being inputted into CARES and IMA including regular reminders to staff about any follow-up required with clients and makes sure that data is entered into the systems by the end of each month.

  • Participates in weekly community meetings with the clients to discuss topics of interest.

    Qualifications:

  • Bachelors Degree

  • 3 to 5 years relevant experience.

    All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
