Veterans Affairs, Veterans Health Administration Program Analyst (PCMM Coordinator) in New York, New York

Summary The New York Harbor VA Healthcare system is currently recruiting for one Program Analyst (Patient-Centered Management Module Coordinator) for the Primary Care-Patient Aligned Care Teams (Pact) service at the New York Campus location. Responsibilities Major Duties include but are not limited to: Ensures data input in primary care systems is accurate to properly determine future needs of staff, PACT teams, and capacity. Reviews the identifications of PACT, the associated FTE, and space used as well as the management of patient panels through the mandatory and consistent use of primary care systems. Awareness of the program needs and train and assist all users in primary care systems locally, the functionality and capabilities, and provide them with analytical background and recommendations. Data collection, monitoring and maintaining provider profiles, monitoring panel sizes, oversight and collection of transmission errors, and review/rectification of incorrect patient assignments. Prepares National, VISN, and Medical Center level reports, upon request using established computer programming routines. Collects, complies, analyzes, and presents data regarding the integrity of the PCMM Package. Assures training, implementation, monitoring, and evaluation of all administrative procedures related to PCMM operations and is the facility resource on all issues that related to the PCMM and the management of this critical database. Primary responsibility for establishing and managing PCMM teams and positions set up for all primary/PACT teams. Gathers information from the PCMM program and enrolls patients and selects the appropriate provider with the PCMM system. Responsible for maintaining the integrity of the database within the PCMM system. Maintains accurate enrollment information, conducts quality assurance reviews of the system data and corrects and notes discrepancies, disenrolls and reconciles veterans from multiple specialties and consult clinic to ensure proper calculations of enrolled veterans and staff within database. Performs workflow assessments and optimizations and makes recommendations in clinical, administrative, education, and evaluation domains as needed. Collaborates with team leaders, Clinical Managers, Providers, Nurses, Clerks, and the rest of the clinic staff, and is responsible for maintaining the PCMM database accurately and timely. This includes routine, ongoing contact with national VHA PCMM meetings, committees, and conference calls. Assist or provides training, implementation, monitoring, and evaluation of all administrative procedures related to PCMM operations as well as work closely with appropriate staff and facility leadership serving as a PCMM resource person. Plans and makes recommendations for future resources in primary care to care for patients. Troubleshoots and coordinates processes with all providers and clerks involved with PCMM. Analyzes workflow to determine potential problem areas and makes recommendation to correct these areas and to improve coordination. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-7 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: experience applying analytical and evaluative methods and techniques to issues or studies concerning the efficiency and effectiveness of a clinical program; experience planning, organizing, and developing studies or projects, making recommendations on operating programs and negotiating with others to accept and implement recommendations; and experience evaluating and making recommendations regarding health care delivery systems. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a master's or equivalent graduate degree OR two full years of progressively higher-level graduate education leading to such a degree that is related to this position. NOTE: Your education must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent one year of full-time study; if that number cannot be obtained from the transcript, 18 semester hours will be considered as one full academic year of graduate study. An equivalent combination of specialized experience and graduate education as described above may be used to fully meet the requirements of this position.
