Job Description

Responsible for the order entry of prescriptions, preparation of intravenous solutions, disposable supplies and related activities under the direction of a pharmacist for intravenous homecare patients.

Education Requirements

High School graduate or equivalent.

Experience Requirements

  1. Minimum of one year experience in home infusion pharmacy preparations for specialty pharmacy home infusion products and parenteral/enterals, intravenous antibiotics and chemotherapeutic agents administered in the home.

  2. Successful completion of a technician training program, or equivalent experience

  3. Home infusion pharmaceutical quality assurance experience with parenteral products

  4. Excellent aseptic technique must be demonstrated on an ongoing basis

  5. Knowledge of basic drug terminology, pharmaceutical evaluation, pharmacology and pharmaceutical law

Responsibilities

  1. Prepares specialty pharmacy home infusion products and compounds sterile parenteral/enteral pharmaceutical and chemotherapeutic agents, as applicable,for home infusion patients under the Pharmacy Manager’s supervision, demonstrating a commitment to excellence.

  2. Performs accurate prescription order entry processes that are held for the Home Infusion Clinical Pharmacy Manager’s review and authorization.

  3. Handles the coordination of collecting essential medications, supplies, and Durable Medical Equipment (DME) for the patient’s treatment in the home,

  4. Prepares labels and affixes to dispensed product as required. Prepares fulfilled medications for review by the Home Infusion Clinical Pharmacy Manger.

  5. Initiates and completes delivery forms

  6. Attends and participates in all home infusion patient care meetings and conferences.

  7. Prepares, records and monitors results for pharmaceutical quality assurance testing.

  8. Assists in maintaining the home infusion pharmacy facility in adherence to Policy and Procedures for achievement of excellent audit results.

  9. Performs inspections of the home infusion pharmacy, checking of refrigerator temperature and documentation, therein.

  10. Routinely checks for and removes expired home infusion medications from inventory.

  11. Responsible for knowing and adhering to Policy and Procedures governing the Home Infusion Pharmacy operations.

  12. Assists Home Infusion Clinical Pharmacy Manager and Warehouse/DME Associate in inventory procedures including record keeping and ordering of home infusion medications, supplies and DME.

  13. Logs and shelves incoming inventory for home infusion operations.

  14. Returns reusable medications, supplies, and DME to stock before the end of the shift, including processing of controlled substance returns.

  15. Assists in reporting and resolving controlled substance discrepancies.

  16. Completes audits of controlled substances as directed.

  17. Organizes assigned workload to best facilitate its accomplishment.

  18. Maintains the confidentiality of patients and their personal information at all times.

  19. Under the supervision of the Home Infusion Clinical Pharmacy Manager and after appropriate training, obtains medication information/history from home infusion patients and/or their caregiver for input into the electronic medical record.

  20. Conducts all required functions under the direct supervision of the Home Infusion Clinical Pharmacy Manager.

  21. Demonstrates effective communication with intake, pharmacy, nursing agency, warehouse / DME staff and other services in person and via telecommunications.

Additional Responsibilities

  1. Assists the Home Infusion Clinical Pharmacy Manager , when requested, in all duties and functions according to standard practices of the profession and state, federal and other applicable laws.

  2. Performs other tasks as requested by pharmacists on duty and supervisor.

Compensation Statement

Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $41.9365 - $41.9365 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
