Job Title: Family Nurse Practitioner Position Type: Fulltime Location: Brooklyn/Queens, NY Pay Rate: $600 - $700 per day. Pay Rate: o The salary range for the Nurse Practitioner (NP) position is $600 - $700 per day. Location: These positions will be based in Brooklyn and Queens. Minimum Qualifications: o For the Nurse Practitioner (NP) role, candidates must be board-approved and licensed in the state of New York. FNP (Family Nurse Practitioner) Job Description Patient Assessment and Diagnosis: o Conduct thorough physical examinations, medical histories, and health assessments for patients of all ages. o Order, perform, and interpret diagnostic tests, including laboratory tests, Xrays, and screenings. o Develop differential diagnoses and identify health problems based on patient symptoms, medical history, and diagnostic findings. Treatment and Care Management: o Develop and implement individualized treatment plans for acute and chronic conditions, including prescribing medications, therapies, and lifestyle modifications. o Manage the ongoing care of patients with chronic illnesses such as diabetes, hypertension, asthma, and cardiovascular conditions. o Provide routine and preventive care, including immunizations, screenings, health education, and counseling on diet, exercise, and smoking cessation. Health Promotion and Preventive Care: o Provide education on health maintenance, disease prevention, and healthy lifestyle choices. o Conduct preventive screenings (e.g., cancer screenings, cholesterol checks, immunizations) and provide counseling on wellness and health promotion. o Educate patients and families on managing chronic conditions and improving quality of life. Collaboration and Consultation: o Collaborate with other healthcare professionals, including physicians, specialists, nurses, and social workers, to deliver comprehensive care. o Consult with supervising or collaborating physicians regarding complex cases, as required by state regulations. o Make referrals to specialists and other healthcare providers when necessary. Patient Education and Counseling: o Offer counseling on various health issues, such as sexual health, mental health, parenting, and lifestyle changes. o Educate patients and their families on medical conditions, treatment options, and self-care strategies. Documentation and Record-Keeping: o Maintain accurate and up-to-date patient records, documenting assessments, diagnoses, treatment plans, and patient progress. o Ensure compliance with healthcare regulations and documentation standards, including patient privacy laws (e.g., HIPAA). Professional Development and Continuing Education: o Stay current on advancements in medical practice, treatments, and evidence-based guidelines. o Participate in continuing education, professional organizations, and certifications to maintain licensure and competency. o Attend staff meetings and contribute to clinic or hospital-wide initiatives
