We advocate for and create affordable housing to end homelessness in our communities."

Purpose Provide case management services to adult heads of households who may have struggled with substance abuse, mental illness and homelessness who are now in shelter or seeking Rapid Re-housing services.

Duties and Responsibilities

  • Create person-centered service plans and meets with clients regularly to encourage, check and document progress, including assisting with personal budgeting, bill paying, and removing impediments to achieving housing.
  • Assist clients in obtaining basic needs using available community resources.
  • Educate providers and market the program as needed.
  • Provide services preparing clients to transition into appropriate CAN 8 housing programs or other permanent housing arrangements.
  • Employ the RRH Best Practice strategies of: screening, and housing-based assessment; Housing search, landlord recruitment, and relocation assistance; Housing stabilization and sustainability supports, including linkages to community services; and financial assistance.
  • Timely and accurately enters all HMIS data to generate requested funder reports.
  • Provide client transportation as appropriate; coordinate and collaborate with other agencies and property managers.
  • Attend required CHI, program and network meetings.
  • Monitor cases and complete all required documentation/charting.
  • Maintain positive communication with landlords. Maintain strict confidentiality.
  • Assist with special projects and complete other related duties as assigned.
  • Other related duties as assigned.

Qualifications, Knowledge, Skills and Abilities

  • Bachelors degree or Associates degree with 3 years relevant work experience.
  • Valid Drivers License with a good driving history.
  • Excellent writing skills required. Able to effectively use the local network of substance abuse, mental health, and social services providers.
  • Familiar with working with homeless populations a must.
  • Time management skills and computer literate with a deadline-oriented mindset.
  • Able to meet all documentation requirements, complete knowledge base of all aspects of the delivery of person-centered case management.
  • Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; capacity to work within a team and foster teamwork.
  • Flexible, able to seek out opportunities for clients to lead fulfilling lives.

Supervised By:Manager of Middlesex Emergency Services Salary Range:$25.50-$27.50/hr. based on experience Schedule:32 hours per week Monday through Thursday 8:00am-4:00pm
