Job Overview:
RWJBH is seeking a Clinical Care Coordinator, Registered Nurse for our Radiation Oncology Department in the Morris Cancer Center at RWJ New Brunswick
The Care Coordinator works closely with the primary care physicians, various specialty physicians, hospitals and ancillary service providers to maximize the health status of ACO patients in their assigned primary care practices
The Care Coordinator focuses on coordinating care and care plans across all in-patient and out-patient settings, facilitating transitions of care, providing patient health coaching, specifically with high risk patients, and facilitating compliance with the required quality metrics for all the ACO patients to ensure optimal patient care and communication of pertinent findings to the patient, their care givers and care team,
Qualifications:
Required:
BSN
Minimum of five years of recent, relevant clinical nursing experience in direct patient care such as ambulatory care, home care or case management
ACLS and PALS MUST be obtained within 3 months of hire date
Certifications and Licenses Required:
BLS
Active New Jersey Registered Nurse License
Scheduling Requirements:
Day Shift
Monday Friday, Some Weekends

Essential Functions:
In collaboration with the primary care physician and health care team identify and manage high risk patients providing an individualized care plan with self-management
goals based upon clinical knowledge in disease management, prevention, health risk, self-care and case management.
Proactive outreach to complex high-risk patients to create and manage their plan of care including medication reconciliation, disease education, and coordination of services.
Contact and manage patients who experience a transition of care in-patient stay or ED visit within 48 hours ensuring the medical, psychosocial and functional assessments are completed after discharge with appropriate follow up is scheduled; monitor that appropriate services and care are in place and being delivered.
Implement population management plans that incorporate risk stratification, tracking for follow-up, best practice guidelines for prevention and disease management and support patient engagement.
Consistently provides comprehensive, community-based, patient-centered, developmentally appropriate, and culturally and linguistically appropriate care coordination and team-based care to patients.
Participate in quality improvement initiatives, tracking and reporting MSSP metrics and implementation of best of breed practices to improve outcomes and utilization of health care resources across the ACO
