Reference #: 2823126 Description Position Summary The role of the Certified Investigator is to conduct ethical, high quality investigations using best practices and widely accepted investigation tools. Certified Investigators will investigate allegations that impact or could have impacted the health, safety and rights of individuals receiving services from RHD. These investigations are completed for the purpose of assuring the health and safety of the individuals we serve and quality improvement as it relates to consumer driven services. Essential Duties and Functions
INVESTIGATION Functions as a Certified Investigator and conducts investigations of allegations implicating RHD staff as it relates to the health, safety and rights of individuals under the care of RHD. Follows the established protocols regarding completion of certified investigations. These protocols include but are not limited to: Conducting at least 6 certified investigations per year (if available) Upon assignment, complete the first witness interview within 24 hours of assignment. Complete all witness interviews within 10 calendar days of assignment, whenever possible. REPORTING AND ADMINISTRATIVE Meet with the Findings and Review Committee to discuss all evidence obtained as it relates to the assigned investigation. Write the Certified Investigation Report (CIR) within 48 hours of completing the assigned investigation. Submit the Final Investigation Packet to the Service Line Incident Manager or equivalent within 24 hours of completing the CIR Unless otherwise indicated, provide all stakeholders with sufficient information to close incidents within 30 days or advise of the need of an extension within 25 days. Attend and lead administrative review as requested Perform other such duties as may be assigned COMMUNICATION Provides regular updates to the National Director of Incidents in writing and verbally Provides clear and accurate communication to all stakeholders around the logistics of the investigation including setting meetings and interviews Adheres to deadlines and provides information in sufficient time to avoid late reporting
Requirements Qualifications include: Credentialed as a Certified Investigator. Unimpeachable ethics and integrity. Knowledge of state/funder regulations related to incident reporting and investigations. Investigative skills, including ability to formulate investigative questions and methods, structure related interviews, review documents and collect evidence, and objectively determine findings and make relevant recommendations. Interpersonal style characterized by professionalism and patience. Well developed and effective written and oral communication skills, Excellent relationship building skills. Ability to use critical thinking and effective problem-solving skills in stressful environments. Ability to maintain confidentiality. Ability to coordinate work within a team environment. Excellent computer skills. Ability to travel to RHD program sites within the division or within other divisions across the entire corporation. If the position being applied to reports out of Philadelphia PA: RHD employees working in programs within the City of Philadelphia are required to provide proof of Covid vaccination. If not fully vaccinated, they must receive at least one shot in a two -dose series or a single dose in a one-dose series before beginning in-person shifts. Final doses must be received within 8 weeks of hire. RHD's vaccine requirements for the Corporate Office located at 4700 Wissahickon Avenue; Suite 126, Building A. Philadelphia, PA: The Corporate Office is not a service location. Therefore, RHD has determined that vaccinations are not required to work out of or to visit the office. As a reminder and as stated in prior communication, vaccinations are required for those who visit or work in Philadelphia programs and/or service locations.
