Position Description

The Metro Public Health Department Bureau of Population Health is seeking a motivated and independent individual to act as our Community Health Worker Supervisor). The Community Health Worker Supervisor supervises the community health worker team of staff and contractors and assists the Community Health Worker Initiative Director with tracking initiative processes, outcomes, and impact. The Supervisor maintains a staffing schedule for community health workers at the Metro Public Health Department clinics and responds to scheduling requests for team members at community events and other ad hoc needs. The Supervisor also monitors community health worker team member performance and may fill in for staff when needed at clinic sites.

Typical Duties

  • Supervises and manages staff supporting patient resource and navigation services in Metro Public Health Department clinics.

  • Evaluates staff performance, coaches and mentors to support skill development.

  • Ensures staffing coverage for all shifts in clinic sites, filling in as needed for coverage.

  • Reviews initial client assessments (PRAPARE) tool and advises staff on appropriate resources with which to connect clients.

  • Supports staff with assisting clients with resource and service navigation, including helping staff trouble shoot resource needs.

  • Works collaboratively with the Community Health Worker Initiative Hub Coordinator to support an external service and outreach schedule for community health events.

  • Works collaboratively with the Community Health Worker Initiative Hub Coordinator to identify and ensure on-going professional development opportunities such as Community Health Worker certifications.

Minimum Qualifications

  • Bachelor’s Degree in a science, public service, or education related field

  • Three (3) years of full-time employment in a related field.

  • Experience in program management, development, or related work.

Valid driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.

Preferred Experience, Knowledge, Skills, and Abilities

  • Experience in outreach and providing education to priority populations.

  • Master of Public Health, Social Work or Public Administration Candidate (completion of some graduate course work)

  • Experience managing staff or work teams.
