Venture Together DSP - COMMUNITY HABILITATION/RESPITE in Nanuet, New York
DIRECT SUPPORT PROFESSIONAL (DSP)- COMMUNITY HABILITATION/RESPITE: JOB SUMMARY: Direct Support Professionals who provide Community Habilitation and/or Respite Services perform a wide variety of tasks that involve both encouraging and supporting Developmentally Disabled individuals to develop independent living skills and to provide access to community resources while providing protective oversight of their health and safety. Direct Support Professionals providing Community Habilitation and /or Respite Services are expected to be proficient in DSP Core Competencies including person-centered and individualized service delivery. The Direct Support Professional Community Habilitation/Respite reports to the Client Coordinator. QUALIFICATIONS: Experience working with people with developmentally disabilities preferred. May need to drive. Required to successfully participate in agency training. RESPONSIBILITIES: Be punctual in arriving for work, trainings, and other specific appointments and dress appropriately. Provide services as indicated in the persons Staff Action Plan, including all safeguards. Encourage and support individuals to become as independent as possible. Encourage Individuals to participate in decision-making including choosing and planning activities. As required, set up and participate in community outings. Complete all required documentation accurately and on time. Develop positive relationships with all individuals that you work with, as well as the individuals families, guardians. Maintain good communication with all involved. Know emergency actions (ie. calling 911) and be able to carry them out in a calm, orderly manner. Assist individuals with skills and activities of daily living as needed. Must be able to walk, stand, bend and stretch for long periods of time. As required, transport individuals to appointments. Be an advocate for the individual and when in public act in manner that will reflect well on them and Venture Together. Effectively communicate with supervisors, individual, their families. Required to accurately document service delivery via an Electronic Visit Verification system.
GENERAL RESPONSIBILITIES: Adhere to Venture Togethers Employee Handbook and Corporate Compliance Plan. Abide by the provisions of the Code of Conduct as well as Part 633 and Part 624 regulations. Complete and actively participate in all required agency training. Recognize the importance of reporting (orally and written) situations and/or incidents to supervisor; required to report all suspected abuse and neglect to the Justice Center. Be able to make sound conclusions/decisions and when necessary, take effective action independently. Be able to accept necessary supervision and direction with appropriate responses and attitudes. Be able to accept criticism and utilize constructive criticism for self-appraisal and improvement of skills and relationships. Able to handle confidential information appropriately and to exercise care to safeguard proprietary information. Use discretion in discussing any matters related to the individuals in the program. Be able to follow through on directions with a minimum of supervision. Staff are required to speak English while they are performing job duties for operational and safety purposes. Responsible for effective use of Agency supplies, equipment, vehicles, etc. and must be prompt and accurate with receipts for monies distributed. Any other duties assigned by supervisor.
