In this role, you would provide advocacy, education, assessments and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care. You would also coordinate with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes. This position will work primarily in the Emergency Department, providing quality care for ED patients and their families, and supporting ED clinical staff.
Additionally, you would provide a range of interventions for patients, families, and caregivers, including counseling, crisis intervention, and resource referrals, actively participating in care coordination for complex cases and collaborating with healthcare team members to optimize resource allocation. Schedule: This part-time position covers our Nampa ED/Inpatient units in the evenings.
MINIMUM QUALIFICATIONS
Licensed Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred.
All colleagues must provide licensure or proof of application in process for the secondary state within 6 months from hire date.
Master’s Degree in Social Work required.
Two years of experience in hospital setting or medical social work setting preferred.
Experience in case management, addiction services, mental health, or population health management desired.
