Intermountain Health Clinical Pharmacist Ambulatory Anticoagulation IMC in Murray, Utah

Job Description:

The Pharmacist-Clinical will demonstrate consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective and cost-effective pharmaceutical care in the scope of their particular assignment. This position will be responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.

This position will be based at the outpatient Anticoagulation Clinic at Intermountain Medical Center. This clinic cares for about 1,000 patients on warfarin therapy and is expanding into caring for more patients on Direct Oral Anticoagulants (DOACs). Some of the care provided includes initiating patients on therapy, managing those on chronic therapy, and providing peri-operative anticoagulation plans. They also provide on-call coverage for patients and this position would be in this on-call rotation. The clinic is open Mondays thru Fridays with no weekend or Holiday shifts.

During orientation and training the shifts will be Mon-Fri from 8am - 4:30pm. After orientation and training, the caregiver will have the option to continue with 8hr/day shifts 5 days a week or alter the schedule to something like 10hr/day shifts 4 days per week.

  • Participates as an essential member of the multidisciplinary team to optimize individual patient medication therapy. Provides comprehensive medication management under established collaborative practice agreements. Monitors physician prescribing patterns to ensure safe, correct, and appropriate use of medications. Utilizes principles of population-based medicine to ensure quality outcomes. Monitors, documents, and evaluates patient response, drug effects, and adverse medication events.

  • Provides accurate drug information to other healthcare professionals and patients to assure safe and appropriate use of medications. Serves on committees as a resource on medications and/or pharmacy policies as required.

  • Actively participates in and leads department cost savings and quality improvement initiatives. Takes responsibility for assuring quality improvement projects are developed, implemented and completed. Develops standard operating procedures and protocols as needed to meet department needs. Participates in organization surveys (Joint Commission or other applicable regulatory agencies) or audit inspections.

  • Trains and supervises pharmacy technicians, pharmacy students, PGY1/PGY2 residents, and other supportive staff as needed. Participates and supports the training programs and mentorship of students and residents.

  • Is knowledgeable and compliant with State and Federal laws, regulations, pharmacy policies, and standard operating procedures, formulary and pertinent Clinical Practice Guidelines.

  • Provides professional, engaging service excellence to all patients and healthcare providers with whom they work.

  • Provide initial clinical evaluation and patient education prior to initiation of specialty medication(s)

  • Collaborate with other pharmacy team members to provide clinical support for specialty medication access

  • This is a 40 hour a week position, that will work Monday - Friday, no weekends or holidays.

Minimum Qualifications

  • Current license as a Pharmacist by state in which one practices.

  • Controlled substance license.

  • Basic Life Support Certification (BLS) for healthcare providers.

  • Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.

Preferred Qualifications

  • Experience working with anticoagulant medications.

  • Completion of a PGY1 Pharmacy Residency followed by a PGY2 Pharmacy Specialty Residency or three years’ experience in an institutional or ambulatory care pharmacy environment beyond PGY1 residency training.

  • Board Certification (BCACP, BCPS, BCCP, etc.) or other added qualifications (CACP, ASHP Anticoagulation Certificate, etc.)

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$59.27 - $87.11
