Westchester Community Health Center Community Health Worker in Mount Vernon, New York

Job Summary:

Under the direction of the Program Administrator or Designee, the Community Health Worker is responsible for conducting community outreach activities and community collaboration to engage and refer community residents. Outreach staff will distribute information about MVNHC throughout the local neighborhoods, at events hosted by local municipal agencies, and community-based organizations. Community Outreach workers will have extensive knowledge of community resources for vulnerable community residents.

Primary Responsibility of the Position:

Develops and maintains professional relationships with community and community leaders, i.e., churches, community action programs, etc. to assist in gaining health center acceptance by the community Presents programs and/or educational forums about the health centers goals and objectives Exhibits strong interpersonal, communication, and problem-solving skills Performs home visits to ensure that all information regarding related health services is communicated to ensure positive health outcomes Informs residents and potential patients about the availability of MVNHC services. Interprets MVNHC services in conjunction with identifying current and potential patients needs for services. Establishes community to ensure that patients are aware of and have access to all suitable programs and services available at MVNHC. Performs related outreach tasks as assigned. Assists and streamlines the registration process of identified outreach individuals. Follow-up with registered MVNHC individuals regarding services rendered. Calls registered MVNHC individuals as a reminder for follow-up or referral appointments. Provides and collects Satisfaction Survey of services rendered. Participates in special community events providing information that promotes the services of MVNHC. Performs other related duties as assigned. Education and Experience:

Graduation from a standard high school or general equivalent diploma One (1) year prior experience related to the duties and responsibilities specified, or five (5) years of professional experience providing outreach services in a health care setting or equivalent combination of education and experience that provided the required skills, knowledge, and abilities. Demonstrated reading and comprehension ability necessary to understand materials. Ability to serve as liaison to community organizations as assigned. Has acquired a full understanding of ethical issues related to patients rights and HIPAA Laws. Must possess knowledge of community support services, and basic requirements of community outreach duties, and have acquired knowledge of the scope and activities of public and private health and community organizations. Must possess working knowledge of computer applications such as Microsoft Office (Windows, Word, Excel) email, internet research. Has the ability to communicate with patients, staff, and associates effectively. Exhibits good judgment, tact, courtesy, and integrity and is in a physical condition commensurate with carrying out the duties and responsibilities of this position.
